Painful periods and headaches

I am 48 and have been having really painful and longer periods for the past year. I take strong medication for migraines and can't take anything hormonal due to this (I can't take any birth control etc due to this but DH has had the snip).
I am also having bad headaches (not migraines) during my period that I didn't have before.
What are my options? I do take plenty of vitamins and pain killers but I can't go on with the level of pain and feeling so rubbish every month.

The Mirena coil has very low dose progesterone, can you have that?

I don't think I could. My migraines are really bad - I take 3 different medications every day and had to stop any birth control I am at the point where I would have a hysterectomy if I could.

PCOS is one of the reasons for hevy and painful periods. Core exercises like side planks, curl ups, and anything that focuses on the stomach are helpful to manage period cramps. Also, chamomile tea is good to manage headaches and migraines.
