Anyone else had Mirena for a while and then added in oestrogen gel?

I'm 45, have had Mirena for 2 years for very heavy periods, now trying to decide whether I should start taking HRT (i.e. oestrogen gel as I already have the coil in).

I have no hot flushes and hardly any periods due to coil so it's very hard to use these as a test for whether I need systemic oestrogen.

My main concern is sexually related - I suffer pain on intercourse and can barely orgasm any more. I have also been using Ovestin for 2 years, it really helped initially but now things have got bad again.

So is the local oestrogen not cutting it anymore? Would systemic ostrogen help with sexual function?

When I got the coil put in I put on quite a bit of weight out of nowhere and was extremely emotional for about 5/6 months but then things did quickly settle back to normal

Will I have to go through all this again as I would now be adding in oestrogen? Or is it the progesterone (or rather fake progesterone) in the coil which tends to be the culprit??

I am starting a new job next month and would rather not subject myself to months of side effects but equally I am aware that the fanny issues could be 'the canary in the coal mine' and I need to add in more oestrogen now before it's too late?!!

Asking her as can't even get a phone appt with a GP for 5 weeks to discuss
