Sore nipples and bleeding

I've been on oestrogel and progesterone since July this year .
I'm upto 4 pumps a day and 2 progesterone 1 vaginally 1 orally and testogel was added about 8 weeks ago. Since upping the dose my nipples are sooooo sore and painful and I'm getting a brown discharge when wiping at the toilet most days .
Any ideas what's the issue here ? The progesterone or the oestrogel

I've had a scan to check the lining of my womb and it's very very thin so no concerns there
