Hi... I'm embarassed to admit i have brain fog and a lack of concentration recently. I've messed up a bit at work and my manager wants to talk to me about it.
What can i say that doesnt sound pathetic and like an excuse? I've been there 10 years ago is only a recent thing...
My manager is a man too... younger than me

Do you have a workplace menopause policy? Are there any adjustments work can make to help you? Would be a good time to talk to him about some adjustments to your role perhaps.

Are you adequately medicated for it? I struggle with brain fog at work, even at top of dose.

Be honest. Agree with PP, do you have a menopause policy / menopause champion / occupational health dept.

Tell him the truth!
A lot more companies are becoming much more understanding of issues arising due tk menopause.
Check with HR if you have any specific policies.
