HRT has stopped working.

I'm 56 years old. Started taking HRT a year ago following about 4 years of symptoms ( night sweats, brain fog, dry vag, no sleep, anxiety etc). No periods for 18 months before starting HRT.
I have 2/3 pumps of Oestrogel in the morning and 100mg Utrogestan in the evening. No breaks.
Initially this worked, my mood lifted, no night sweats so better sleep. I was very pleased!
However, last month I realised I was feeling anxious and had several bad nights. Then my breasts really ached and hurt. Then 7 days ago I started to bleed. Looks like a proper period and it is only just slowing down. I also have a migraine which was a normal period symptom for me.
So, it seems like the HRT has given up.
A recent ct scan ( for gastro problems) showed a fibroid.
Should I be taking HRT with a fibroid?
How worrying is the bleeding post menopause?
I'll try to get an appointment with my GP, but as there is a 4 week wait, I thought I'd seek some advice here in the meantime.
Help!

You probably need to up your oestrogen, so you need to speak to the GP first. And you’ll be needing a scan. I’d get onto the receptionist as you should be seen under the two week rule.

I'm sure your meant to be able to get a appointment within 2 weeks now but they obviously can only give you what they have. I agree with above poster you just need to up your dose. I'm mid/late 40's and its took me 9 months to find the right hrt, same strength just different methods , now on a patch.

I started to bleed when I started hrt . Went to doc after about 8 MTHS of period like bleeding. Sent to hospital to see gyn. He increased my progesterone to double and sent me for internal as scan showed a small likely polyp. Had internal and they removed benign polyp.

Gyn told me if I wanted I could increase my oestrogen which I did briefly to attempt to deal with symptoms. I have continued to bleed (lightly) every three weeks or so. Doc seems unbothered and said it will settle but also offered me coil.

Not sure if my little story is of use but shows you I guess that bleeding can be benign. Your case is different as it started after a year. I do suspect we don't fully know how hrt interacts with our bodies but you need to check it out to rule out more serious causes. It's possible you might just need to increase your progesterone..
