Dealing with time zone change

I'm heading to USA on holiday and worrying about the time change for taking my HRT. I'm on tablets.

I've been on HRT since December following a hysterectomy. I've missed a dose once and taken another dose late and both times I got awful migraine.

Has anyone changed the time for taking the tablets? Will I need to just suck up setting an alarm for 3am so I'm still taking it at the same time?

The thoughts of a 4 day migraine on holiday is giving me anxiety.

I’m also off to the USA on holiday, I was planning on just shifting to that time zone when I’m there and moving straight back again, but I guess you could try shifting it a bit later gradually at home, adding on an hour a week maybe?
