Please help me understand cycle on hrt

I am not sure whether this a problem or not but I can't get my head round a change in cycle.

i am on Evorel 100 (prob far too high) and 200mg of Utrogestan for 14 days. I was still having regular periods a year ago when I started HRT (who knows now).

Cycle used to be
Days 1-14 Evorel
days 14- 28 Utrogestan + Everol then a bleed 1-4.

Bleed now starts in the middle of taking the Utrogestan - so days 20 to 24.

Is this okay? Is it normal to have a period/bleed while taking the progesterone? I am
confused 🫤. Dr was no help and v little idea.

How old are you now?

As a guess, it's probably because the Utrogestan isn't enough to control your lining because you are either still ovulating and producing your own estrogen. Or, you aren't absorbing enough Utrogestan to control the lining on 100mcg patch.

In some women on 100 ( or more) there's been a suggestion to increase the Utrogestan to 300mgs .

@PJ04JCW I'm not really sure what you are doing or asking (sorry!) but the advice is to start Utrogestan on the same day each month and stick to it. Your own natural cycle is no longer part of it all. HRT sets an HRT cycle.
So you can't use your own cycle. eg you could start Utrogestan on 18th May and then 18th June etc.

The leaflet is a bit confusing as it implies all women have regular 28 day cycles which is simply not the case especially in peri. Your GP ought to have explained how to use it.

@JinglingSpringbells that makes a lot of sense, thank you for replying. I am 48, and I think (a certain private clinic) upping the dose was a mistake, which feels scary. I really want to come off it all and start again, it feels very odd not knowing my "real" cycle or where things are at. My cycle used to be/is like clockwork despite all the other peri symptoms so this has been disconcerting.

But thanks again, really grateful for a considered reply.

@JinglingSpringbells that helps thank you! GP told me to start the HRT first day of my period but maybe she meant just the first time. Thanks

I thought it was 200? I increased thanks to a friend who is a Gp suggesting it after BMS training.

Hadn't read 300. Though I think I could cope!
