Using Utrogestan vaginally

Wheretheskyisblue · 17/04/2023 09:14

I need some information about using utrogestan vaginally as my GP has no idea and has never heard about this.

  1. I was wondering if I need special capsules or whether I can just use my normal ones?
  2. I am currently using 200mg for the first 12 days of the month, 2 capsules, do I use the same dose or can it be lowered for vaginal application?
  3. I get my period normally around day 10. Can the capsules be inserted before I put a tampon in?
  4. The reason for me trying vaginal application is acid reflux. Has anyone experienced improvement of symptoms of this with vaginal application?

I have always used mine vaginally (mainly because I suck at taking tablets orally)

I insert two tablets instead of swallowing them - the dose remains the same.

You may find that your period is delayed until you have a break of them by doing it this way, this is what happened with me.

One word of advice is to put them as high up as you can.

Hope that helps!

1 You can get 200mgs capsules that are specifically for vaginal use. However this is off-label when used as hrt because the 200mgs are for fertility treatment. So unless you have had them prescribed off-label, (unlikely by a GP) you will need to use 2 x 100mgs.

2 Use the same dose (not less)

3 Your own cycle should be overridden by HRT and you may find you do not bleed until 3-4 days after the last capsule.

4 If you do bleed before using 12 days, take the remainder orally.

5 You can use them vaginally at any time of day (doesn't have to be bedtime) but put them high enough so they don't fall out. You might get some leakage of them to be warned!

Ok thanks both. I read somewhere that you get better absorption vaginally, hence needing a lower dose, is that not the case?

Also does anyone know why the dose of utrogestan is always 200mg and does not change dependent on the oestrogen dose?

I’m on utrogestan pills, 100mg, 12 days per month at the end of my cycle.

Wheretheskyisblue · 17/04/2023 15:13

@Sewingdufus Can I ask why you are on 100mg not 200mg? Is it a private prescription?

Just starting out, taking it orally (apologiesI should have made that clear). No idea why 100 not 200, that’s the dose I was given by nhs GP.

JinglingSpringbells · 17/04/2023 15:35

@Sewingdufus That dose is wrong.

If you are using it for 12 days, the dose is 200 mgs - 2 capsules. You need to go back to your GP. Also, the dose is stated on the patient leaflet in the box of tablets. The only way you would use 100mgs is if it's daily, every day.

This is the same dose orally or vaginally.

@Wheretheskyisblue The British Menopause Society has issued an update - over a year ago- emphasising that no dose reduction is allowed for vaginal use.

hi I’m so confused! My doctor agreed to taking utrogestan 100 capsules vaginally, I was having terrible side effects taking it orally. He said to continue to take x2 100mg capsules every night for 14 days on 14 off.
ive been doing this for 3 nights and my god do I feel amazing!!! However last night when inserting there was a large lump just inside of my vagina that wasn’t there the previous night? I’m so scared and not sure if I should continue taking this way! Can’t get an appointment until 5th March? advice will be greatly received. X

@Amelia78 It probably completely normal. The vagina has all sorts of ridges and folds (up and down, and across) sorry for TMI! It might feel different everyday but unless you were exploring it daily (!) for any reason you're not going to be aware of how it feels.

If it's just inside it's unlikely to be a prolapse- which would be your cervix dropping right down - and you should be able to tell if it's that are the walls.

If you've had any signs of prolapse before, see your GP but otherwise there probably isn't much point and whatever is there might not be there a few days later.

But obviously see your GP if you are concerned.
