Cycle messed up advice needed re: bleeding and Utrogestan use

Backstory: started HRT April 2022. Estradot 50 and 200 oral utrogestan for 12 days taking 1st dose on 22nd of the calender. Nov 2022 Estradot out of stock get prescribed Everol 50. Dec and Jan period goes wonky (Dec lasting days only and nearly 3 weeks in Jan). Manage to get Estradot in Jan (13th to be precise so switch back).

In Feb cycle starts around predicted date, takes utrogestan on 22nd as usual for the 12 days. Nothing odd in this month.

Except now it’s March and I’m due to start utrogestan tonight (I log everything on Balance App) and I started spotting yesterday only 2 weeks since last cycle ended and woke at 4am in full heavy flow with the Worst cramps, night sweats (which had gone completely!!!, and palpitations (anxiety about what’s happening??). So I’m awake and confused and worried.

Q) Do I take utrogestan as expected tonight for the 12 nights and put this down to erratic breakthrough bleeding?

Q) do you get cramp with breakthrough bleeding?

Q) Has my “monthly bleed” been breakthrough bleeding or an actual period? Some months it’s heavy and causes cramp other months it’s quite light and pain free.

Q) should I call my GP or is that overkill? Plus getting even a phone consultation is like getting blood out a stone (excuse the pun). Should I speak to a pharmacist??

Added info: I was diagnosed with fibroids in 2004. I had heavy painful clot filled periods often prior to HRT. Started it for perimenopause symptoms despite being “regular”. These included (and have since gotten so much better or gone completely) night sweats, anxiety, itchy ears, itchy eyes, brains fog (it’s only slightly better), itchy skin, dry down there, palpitations, sleep disturbances, headaches, mood swings (marginally better) and tingling of fingers and feet.

I assume both patches are just estrogen? If so, swapping to a different brand, the same strength, shouldn't affect your periods.

It sounds as if it's your own period breaking through.

I think I'd wait for the bleed to finish, then whatever day that is, re-set the cycle EITHER by counting 15 days (so you start Utrogestan mid-cycle, in theory) OR you start it on the day the bleed finishes.

I appreciate it's very inconvenient to have the bleeds, but from a safety point, it's not really a huge issue as it does sound as if you are having your own periods that are not being controlled by Utrogestan. Maybe use 14 days of it which is an option for women who bleed too much?

Yeah the patches are just Estrogen.

I called my local pharmacist who Googled something and told me to just take it from tonight as planned and see what happens, not exactly filling me with confidence. She also said I could halve the dose to 100 and take for 25 nights and a break for 3 nights but she was just reading this off t’internet and I’m not convinced I can do this in peri. I called GP and I’m currently awaiting a call back so watch this space.
