Two years on low dose HRT and heavy cyclical bleeding

I can't, just can't bear the thought of having to come off hrt. But after two years of sandrena (.0.5)gel and Utrogestan 200 for 12 nights, my periods remain and are getting heavier, I've changed four thick pads since this morning.
I'm 52. In 40s had flooding for periods, small fbroid, uterine polyps plus mild adnemyosis.
Had polyps removed 2018, periods calmer but bladder in hell. Local hrt never worked. Started systemic in 2020 plus estring in too. This helped bladder but polyps were back with a vengeance and uterus very thick.

Removed estring and had hysteroscopy for polyps in Dec 2021. All OK till two months ago, periods are much heaviee with clots.

I know I'm not meant to be on HRT. Every time I have a mammogram I'm told not to touch it as my breasts are so "active".

But I was literally suicidal after five years of bladder hell.

Has anyone continued having heavy periods through hrt? Is Ablation an option do you think?
I'm trying to see a gynae but it'll take centuries.

what about the Mirena?

The other option is to use 300mgs utrogestan but it's unusual you are bleeding so much on 0,5mgs estrogen.

JinglingSpringbells thank you. Mirena not an option for me because of another problem.
Looks like it needs investigating. My mum had her uterus taken out for similar reasons, my maternal gran plagued with heavy bleeding. With my bladder history no one wants to touch my uterus.
I'll try to get an appointment with my GP
