Horrendous cramps

Hi All, I'm after a bit of advice.
Most of my life I've had relatively pain free periods - in fact they came and went and I barely noticed many.

I'm now 50 and clearly perimenopausal. I've starting to have severe period cramps. I'm talking sitting crying on the loo - thinking I have to go to A&E pain :( It lasts about 24 hours.
GP has said this is likely a PM symptom but I'm after some advice how to manage. So far best relief I've had is using a heat pad/water bottle but can only do that when I'm home really.

I already take various pain killers regularly for a frozen shoulder I have so I'm not sure what other medication I can try.

I tried HRT (gel and patches) a few months ago but they made my migraines much worse, so I gave up.

Any suggestions please - it's potentially causing me to have sick days off work and I cant go on doing that once a month :(
