I absolutely dread my periods arriving these days?

I have always had heavy periods although when I was younger apart from the heaviness, my periods were not a huge issue for me, no PMS or any issues during the actual period.
But that has all changed over the last few years and it’s getting worse.
Over the last 7-10 years I keep getting uterine polyps and have a constant thickened uterine lining (approx 18mm). I have had the polyps removed 5 times but they keep growing back. They were last removed in September 2021. I have just had another US scan and they are back again!
I am on a waiting list to have an ablation but due to extreme anaemia I can not have the op until my levels are higher.
I am 49 in a few weeks and am starting to get awful PMS, something I never used to have. I fly into rages, one minute ok, the next bawling my eyes out, anxiety which is beyond anything I’ve suffered from before. My back and hips ache, digestive issues, I feel tired, broken sleep, fatigue, hot sweats at night in the week before period.
When period comes I have started to get aura migraines on day 1-3 which leaves me with such disturbed vision that I have to stop what I am doing and go into a dark room.
The period is usually really heavy on day 2-4 and I flood and pass large clots.
I don’t know what to do. I am on edge as each period looms, I absolutely hate them.
I haven’t had a proper consultation with a gynaecologist since 2020 and even then it was a 10 minute telephone consult where the gynae barked down the phone his opinion on what HE was to do. Asked me no questions and wouldn’t allow me to speak or ask any questions.
I don’t know if I should pursue the ablation route, try the Mirena or ask for HRT.
All I know is that my hormones must be on a rollercoaster as I feel crap mid cycle too.
Anyone else been through this?

Please go and talk to a doctor as you cannot go on like this. My periods always been heavy first few days but lately while I am in peri meno they are so heavy first 2 days that it floods out of me in the morning. Pmt is awful but try to cut out sugar and drink lots of water. I have been trying to get hrt for years and male doctor would not give it so went to a Wellwoman Clinic and they were great. Am on Divigel (estrogen) gel which I apply daily and then 14 days on progesterone and 14 days off. The progesterone helps me sleep and is great so cannot wait to start that again. Am feeling a bit better already but it does take a few months for the hormones to balance out. But you do need to see a doctor and insist on a consultation with gynae as you have had the other issues. Hope you get something sorted but was so afraid starting hrt but wish had done so years ago as brain fog has lifted and joint pains gone already.
