Confused by GP re increasing HRT dose

I've been taking Kliovance for nearly a year. At first all was good, over the last few months I have started feeling worse again - my anxiety is getting out of hand, I'm very irritable. Saw my GP this morning to see if I could be switched to something with a higher dose - I thought he might put me onto Kliofem. But according to him, no higher dose was available and he's given me patches on the same dosage instead.

Does that seem right?

Has anyone gone privately for HRT help - I feel like GP advice is a bit hit and miss? (Last year I was accidentally prescribed sequential HRT, when I questioned it, the GP was a bit snappy and said my pills were continous as there were 28 of them rather than 21- I went home checked again and phoned back to surgery and another doctor phoned back to change my prescription).
