Peri or PMDD or both?!

Hi,
I wonder if anyone has any experience or advice as I’m at a bit of a loss.
I think I’ve always been highly sensitive to hormone shifts. The combined pill (many types) caused depression for years and really dulled me, I had PND with both my kids which came on like clockwork overnight at two weeks both times. Pmt has gotten worse with age.
I’m 40 and in the last 6 months or so I feel things are deteriorating and having logged my symptoms for the last three months my quality of life just feels rubbish! I’m not sure if this is the start of perimenopause or its PMDD or both?
My symptoms aren’t classic PMDD as they’re not just from ovulation onwards, I get bad anxiety just as my period ends for a few days which is when you’re apparently getting more oestrogen and people feel good! Which is why I feel maybe this is peri? I get really angry/rage filled at times, low stress tolerance, occasional insomnia, occasional days of extreme fatigue, anxiety after period ends, very low days/depressed. But then some days I am ABSOLUTELY FINE!
I have the Mirena coil which I had put in about 18 months ago but I still ovulate just light periods.
My GP is going to refer me to a gynaecologist but she said I need to see the right one or it might be a waste of time. I want to feel armed with what might be best to try and to have some idea of what’s going on. Blood tests obv revealed nothing! Any advice?
