coil and gel

What do you think of the coil and gel combo?

I'm 2 year post period pand went to get HRT advice from doctor, who is a women health specialist

This is what he's recommended. I'm a bit scared of the coil.
he said it was low risk.

He also said I could try the oestrogen alone for three months to see if I liked HRT and if I did I could have the coil fitted.
he said three months of oestrogen wouldn't harm me.

What do you think?

Yes choc, it’s Normal particularly this soon as it takes up to 6 months to work properly. Even then It can cause bleeding (see my earlier post). It could also be off the back of the hysteroscopy.

If you have a gyn follow up number I’d call themmjust to be 100% sure all is ok thoUgh.

Hope it settles ok.

Hi everyone.
At present I have a coil fitted and have oestrogen gel of which i use about 2 pumps a day.
All was good but the last 2 months I'm having periods again, they're even monthly like a young persons cycle.
I have no hot flushes but do have aching bones which i can live with.
I originally paid private but the doc made me so poorly with terrible bleeding and clotting and then abandoned me (felt like all she was interested in was my money)
Anyway my doctor took over because she felt so sorry for me, I've had all the scans, womb all good, breasts all good, but I just can't seem to get to the stage where other women say they feel fantastic since HRT?
As you can imagine i don't want these dreaded periods back and they come with terrible pains that go straight through to my back.
I don't know what to do anymore, I've been battling this for about 5 years now but apart from hot sweats the HRT isn't really working or certainly doing what it's supposed to do.
I expect to feel tired etc I've gone off alcohol too which is great but what's the point of the HRT if I'm bleeding monthly?
My doctor will give me a home call next week but any advice would be really welcome.
Should i up the gel to 3 or 4 pumps or down to 1 pump?

How old is your coil? It IS a Mirena and not just a copper one?
The Mirena last for 4 years as part of HRT.

How old are you and what stage o f meno were you at when you had HRT?

It sounds as if it's your own natural cycles breaking through again or the Mirena needs replacing.

I'm 57, It's the Mirena coil, I started HRT originally because of terrible lack of sleep which I've always struggled with but got worse.
I first started the HRT in 2016 and originally was fine but then the symptoms crept back so she changed the meds but it made me very poorly.
They fitted the coil about 6 months ago, little bit of bleeding at first (to be expected) and the last 2 months really miserable periods.
I suppose really the problem for me with taking HRT is the bleeding but it's not spotting it's horrible.
I don't really know what to do anymore I've gone to the trouble of having the coil fitted (have to go into the hospital as had 3 caesareans so cervix never fully dilated) it's a nightmare to fit but I can cope with that.
Was wondering whether to up the gel or cut back?

Sorry I should have mentioned I have fibroids but they have been there for years, the scan I had done recently confirms they are shrinking back nicely.

@slopenose It's quite odd you are getting regular bleeds with the Mirena. Maybe it's your own hormones? You ought not to get any bleeding after 6 months using the Mirena or ANY type of continuous HRT. Do you need another pelvic scan? I'd be asking for that.

The whole point of the Mirena is to stop the lining thickening and bleeding although in women who are pre-menopause, it doesn't always stop periods for all of them.

You will get more bleeding with more estrogen so could try reducing to 1 pump.

What other sort of HRT have you used? Have you tried gel and Utrogestan for example? Or a combined patch?

I started off in 2016 with Tiblone 2.5mg one a day. That stopped working after about 18 months the flushes and other symptoms crept back.
I then had Evoroel patches 75 twice weekly with Medroxyprogesterone 10mg one a day.
That made me really poorly so I stopped taking all HRT for a few months because as I said earlier the doctor(Private) that prescribed it for me gave me no further advice and i was worried it was making my womb thicken.
I had all scans done, all good and the Consultant gave me the Mirena alongside the oestrogel which i took about 6 weeks after the coil was fitted.
I am free from flushes but it's these pesky bleeds that i don't like and frankly it worries me it's doing more harm than good.
I think from tomorrow I will use 1 pump until my doctor calls next Wednesday see if that helps.
I had the Merena fitted before menopause for heavy periods and it was brilliant worked great. I had a big bleed and it fell out around Dec 2014.

Presumably the coil is still there now and hasn't fallen out ?

You might be better off with a natural form of progesterone rather than MPA which you had at first.

But the bleeds with the Mirena are very odd- be interested to know what the dr says.

Yes I'll let you know. Last time with the heavy bleed they thought something wrong maybe wrong but it was only when I started the medication? I'll keep you posted

She has however put me in for a scan to make sure the coil is positioned properly as she feels it could be the issue.
I mentioned going to 1 pump a day with Oestrogen, but she didn't seem to think this would make a difference and advised if flushes etc come back to go back up to 2 pumps a day.
Thought I'd give you feedback in case anybody else gets this problem.

Hi slopnose, I know this is years old but thought I would try and get in touch in the hopes you still read mumsnet! Did you get to the bottom of your problem.. ? I have a very similar experience.. 15 months on mirena, it’s all in place (have had it checked twice) but still having bleeds/periods.. last one lasted a month! I’m so fed up as I know I need hrt (reason I had mirena in the first place) but every time I try oestrogen either gel or patch I bleed more. I have tried various combinations from 1-3 pumps to cutting patches.. nothing seems to do what it’s supposed to.. I’m 49.. so very much in peri still but the frustration is real!
I hope you managed to sort your issue out.
