45, irregular periods but some very , very heavy and very long- taking norethisterone occasionally. Last period, had really bad back pain for around an hour, followed by flooding and pain then gone.

Ultrasound ok, no fibroids. Hysteroscopy results clear, smear results ok.

GP not suggested other tests, not mentioned menopause..

GP recommends mirena.

Should I ask for other alternatives? Any advice?

The Mirena worked like a charm for me, stopped the peri-menopause related heavy periods completely. I was 46 when it was put in & I've only had the occasional slight bleed in the almost seven years since. I'd highly recommend it, but others haven't had such positive experiences so I'd recommend doing your research beforehand.

The Menopause Matters website is a good resource I've seen mentioned on here a lot & you might get some useful info there.
