Bleeding for nearly 4 weeks

I'm 42, came off cerazette 2 years ago and periods pretty much like clockwork since then. Then from January this year they've gone a bit haywire, 5 or 6 weeks between them and they've gradually got longer in duration. This one has been going for 4 weeks now, flow is medium, possibly verging on heavy, with clots but no pain. Am a bit worried about anaemia, feeling quite washed out now and wondering at what point I need to be doing something about it. Assuming this is a perimenopausal symptom would going back on Cerazette be a good idea?
