Where can I download a parenting plan format from?

That's the question, someone passed me the link years ago, but now that he we are reconsidering the agreements I would like to have a look at one again.

I remember there was one printed out by some parenting organism where most regular situations where covered and you only needed to fill it in.

Does anybody knows where such wonderful resource is hidding? Have checked CAFCASS, Gingerbread, and CSA without luck.
