What kind of document would be needed?

I have name changed for this post. I have in fact been a poster on MN for a couple of years (under various different names)

I am wondering if anyone knows what kind of document if there is any that allows a NRP to give up parental responsibility?

So basically something that would mean I won't go for maintenance ever and he won't go for any form of contact with DD or have any right to any say in education, medically, location etc.

I know that this isn't normally an advisable situation and it can be quite an emotive subject but it is a genuine question so any help would be appreciated.

I believe once gained PR can only be removed by a court order. So best guess would be a c100 form (if using england/wales courts).

Child maintenance is not linked to PR but biological paternity.
