Isn't there a phone number to call if you are having trouble filling it in? If you don't have the information then I'm sure they will go through it with you, although I have no experience of it myself, sorry.

hello nutcracker i work in a jobcentre. Dont you worry about the forms. As long as you have had a go, your adviser will go through all the forms with you. Believe you me , we get some that havent even been bothered to fill out name and address, and we muddle through them all. Is there a particular part of form you are stuggling with? I could help if you wish. If not, as long as you are on time, or maybe get there a little early, you never know your adviser may have a few minutes to spare if she is not fully booked up. Dont worry though , no need to.
hth.

Thanks pussycatmomma

I took them in and they did go through them and fill in any bits I missed.
Had to run down to where i used to work and get my final wage slip and given them that and I still owe them proof of child benefit too.
They don't make it easy do they.
