Registering the baby

We're an umarried same sex couple who have finally had a baby using donor egg and sperm. We're going to register him next week. We did all the HEFA consents and forms at the clinic but I can't work out what we need to take with us to get registered as his parents?

I can answer my own question now having had some help in case useful for anyone else - it's the WP and PP form from HEFA that's needed
