Advise on how to start court proceedings

Hi im looking to start court proceedings i domt have much money so will be doing it all myself, I need an emergency order to try and get my daughter back home with her sister after her dad took her 6weeks ago, does anyone know what application form i need?

Weren't you or someone with the same situation advised extensively last time this happened? So maybe searching for that thread may help
