Requesting a change of date for an investigatory hearing

I have been signed off sick for four weeks by my GP due to anxiety and depression I'm now being called into work for an investigatory hearing on Monday.

I don't feel up to attending this meeting can I request a change of date?

You can certainly request it but you need to check the sickness policy to see if it is a right.

I requested that they send me a copy of this policy but was told it is my contract, however I do not have a copy of this.
