Who do i ask to help me formulate consent forms and terms and conditions for my medical business?

I am a Nurse,and I work independently for myself providing a nursing procedure for people in their own home.

I need to provide consent forms and terms and conditions, but I have no knowledge or skills required to do this. Who do I ask, a solicitor ?

I'd contact whichever organisation regulates independent nurses.

I'm a will writer and member of the Institute of Professional Will Writers and they provided me with a template Terms & Conditions letter & lots of other standard forms etc which were v helpful.
