urgent advice needed about maternity allowance and mixed (self-employed/paye) employment

One of my sister's is due in mid December. She has, stupidly, left it really late to apply for maternity allowance and is now in a pickle about what she should be applying for.

She has been on a fixed part time contract (a year) which ends as she goes on a maternity leave/just before baby due. She paid tax at source. She also has additional work and this is self employed and she pays her own tax/NI.

What allowance is she entitled to? What form does she fill out - and is she too late?

If she has been employed within 15 weeks of her due date and was in that employment for longer than 26 weeks then she's entitled to SMP. If she's not entitled to SMP then she can apply for maternity allowance through Jobcentreplus. I found the directgov website really helpful when trying to figure out which I was entitled to last year.
