Quick one about applying to court for holiday contact

DH wants to apply to court for holiday contact. He already has a defined court order with some very limited holiday 'extra' (1 day per week of holidays) but feels that 3 years on, DSS would benefit from longer periods and ideally, the opportunity to come on family holidays abroad.

Does anyone know if we need to fill in a C1 or a C100?

has he asked ex if he can have extra contact? even though it is a Defined Contact Order they could negotiate between themselves and come to an agreement.

Yes - has also arranged, paid for and attended mediation, made suggestions about how to extend contact in a child-centred way. He really has tried every other way.
