Trialling manopour. Any info would be great

Hi! I have just had my 4th failed ivf cycle for another child. Im 41 and in perimenopaus and amh 1. I dont make embryos to get to transfer and if I do get to do a transfer its usually with a very poor quality embryo. I said I wasnt going to do any more cycles as my clinic just kept using the same medication but increasing the dose which clearly isnt working so was giving up. My FS has asked me to try again trialling a different medication. I was previously on puregon but switching to manopour which my FS said is probably more suited to my age and she has had success with other patients my age. She also let me know that I might not respond at all and just to keep an open mind, so its a bit of a gamble. I guess before I actually commit to another cycle, im wondering how others my age with very low amh have gone using manopour or even just switching from one medication to another. Did it really make a difference and did you have a baby?

I only had two rounds on NHS which did not result in baby sadly. Both rounds I was given menopur. The first round, I started on 150 and went upto 225 but was aborted before egg retireval as showed very little progress. The second time around, I was started at higher dose 275, and on day 5 scan again, there wasn't much progress, the nurses still felt it was an under response based on my AMH measured at 26 they were hoping to see more follicles recruited but they increased it further to 275. Things looked better on day 8 scan. On egg retrieval think day 13, there were around 4 follicles that measured 16mm to 21mm. However, only two had eggs, and only one was mature, which didn't fertilize. So in my case, it wasn't a great response, but apparently it's a good drug that helps both growth and maturity of eggs.

My BMI was at 30, I believe this may have made a difference to my body absorbing and responding to the doses, because so many women on here had good response to menopur.

I took Menopur for both my rounds. I’m your age, not in peri-menopause (well I probably am but no official diagnosis). My AMH is higher than yours and I am overweight. First round resulted in nothing to freeze / transfer. Section round we added PICSI and HGH which resulted in one really good grade embryo transfer (which failed) and two average grade embryos which we’ve opted to PGT-A test (not holding my breath for a good one tbh!). So no baby but it is working as a stimulant for me

@overwork thank you for your comment. I really hope on of those embryos sent off for testing is good and you get to transfer. I honestly think my baby making abilities are now behind me, but the medication change is my last option. I dont get enough eggs and we already use icsi. I hace nothing wrong with my ability to carry a child or for an embryo to implant , its just I cant make enough eggs to make a good embryo to transfer. My last close to success was my second round , I got to 7 weeks and miscarried, I havnt been able to get embryos since.

It’s a tough line isn’t it, between oh one more round, this could be the one, and the financial and physical realities of it all

Im really only trialling this new medication because my fs has asked me too but I think I have come to terms that it most likely womt work and im probably wasting my time and money but then I have the "what ifs".

Ask if your clinic will transfer multiple day 3s

My clinic only transfers 1 embryo at a time which is standard procedure for most clinics here in australia. also I dont make multiple embryos to transfer usually non at all to transfer

One blastocyst is standard (though over 40 many clinics will do two) but the day 3 limit is usually higher. Are they arresting before day 3?

Yes or I dont have fertilisation. I only get 1 to 2 eggs at each egg retrieval. The new medication they want to trial are hoping to increase to maybe 3 to 4 eggs and a better quality and hopefully make 1 good embryo for transfer. Or it might be a complete flop in which i wont try again.
