My IVF trigger plan may not suit as a poor responder!

Having abit of melt down not sure if it's the hormones but I just had my day11 scan today and the 8 follicles that were first slow responders, after a higher dose of menopur, have reached:

  • 1x 20mm
  • 2x 18mm
  • 2x 15mm
  • 3x 13mm

So grateful for the improved response and now scheduled in for EC Wed morning (29th).

My clinic told me to take the following today:

  • last stimming Menopur 375 @ 13:00
  • last antagonist Ganirelix @ 17:00
  • trigger Ovitrelle 250 @ 21:00

Later today I realised I've been a poor responder with my first cycle abandoned, and this one requiring a much higher dose to get the follicles moving, likely I would've benefitted from a dual trigger. I looked up previous posts on here and so many have shared that the egg quantity, quality and maturity was so much better on the rounds they had dual triggers. Particularly those that took a single ovitrelle had more immature eggs on EC day. I feel so frustrated as to why they didn't give me dual trigger since im not at risk of OHSS with less than average follicles? I can't even contact them at this point of the evening.

It seems they initially thought "at risk of OHSS" with AMH 26 and PCOS, but only to find out I was completely opposite: a slow / poor responder. Since the first cycle cancelled, I spent hours and hours researching what can be done to optimize my chances for the next, and again this time round, hour and hours, researching every outcome possible ahead of the next scan so I'm equipped to advocate for myself. I truly feel for the women who've undergone this stress more than once , I'm full exhausted of being my own doctor around every corner😩

Is there anything thst be done tomorrow? Ie once I take the the trigger tonight, can I request for another tomorrow? Is that how a dual trigger works split over two nights?

Any advice is truly appreciated 🙏

Bump anyone?

Ended up taking trigger shot last night and cherry on top there was abit left in the syringe as I took it out and for whatever reason I thought it'd be clever and "squirted" a very small amount into the air 🤦‍♀️😩

I don’t know about the first bit, but don’t worry about squiring a tiny bit of medicine out, as long as most of it went in there is a margin for error in the tube, sometimes some is still left in there even with full depression. I got all freaked out becuase, after watching an instructional video, ejected the air bubble (that’s meant to be in it to ensure a full dose).

as to your research for what protocol would work best, my honest opinion is that nobody knows how a person will respond from one to the next. Certainly feels like a guessing game mildly based on research but with so many outliers. Some folk get lucky and make lots of nice eggs, I responded worse on higher doses than low, and my only pregnancy was from the lowest dose stims possible making just one egg. Try not to worry about this round’s doses right now as there’s nothing you can change. Once egg collection has taken place and hopefully a nice selection of embryos are laid out before you, then you can think about how to tweak and change. It’s annoying when it feels we non experts are leading the way with protocol, but I think it reflects the fact that so much is unknown.
