IVF short cycle - am I under responding?

Please can someone help me understand my day8 scan, I'll try and keep it brief:

Day1 commenced menopur 187.5

Day5 scan showed around 7 follicles all below 10mm. E2 level in blood showed 921. Nurse said at this stage under responding but quite normal in PCOS case, menopur increased to 225.

Day8 scan (today) showed more follicles total 11 but still measuring below 10mm, at this point nurse said would've liked to see them measuring around 12-14mm. E2 blood test done again estradiol level has increased to 3,200 which they said is good news and want me to stay on 225 dose with the belief that follicles are responding but catching up to reflect Estradiol. My understanding is that Estradiol increases with/following egg growth and sometimes there can be a lag, not the other way round? Also I did a quick google search apparently day8 E2 level should be around 1,000 and anything higher is risk of OHSS?? Im so confused that's a big discrepancy.

Feeling quite confused and anxious at this point and trying to trust the decion made. Any thoughts are most welcome and appreciated.

Hi, your post was a few days ago so hopefully all good your end but was your estradiol measured in pmol/L or pg/mL? The 1000 on day 8 cut off sounds like pg/ml and your results sound like pmol/L in which case you would still be under as 3200pmol/L is 871.616 pg/mL according to the conversion rate I found online. Fingers crossed!!

@Lavendar01 the nurse saying that the follicles need to catch up to match the oestradiol is weird, as it's the developing follicles that create it.

Just see what your next scan shows. It's more important for them to be similar in size to each other than a particular size by a certain date.

@Waitingforday6 I have a scan tomorrow hopefully better response. God knows how they measuring it first bloods showed 921 and now 3,200 that's all i know. I will clarify tomorrow.

@sirensong yes that's what my understanding is, glad to know I haven't misunderstood it but will also clarify this tomorrow. Atm it seems they are quite varied in size, hopefully they'll catch up. They think stims will continue to Wed 14th and egg collection Fri 16th ofcourse if all looks good Wed.

Just thought I'd update on here incase someone comes across this post in the future.

Went for my day11 scan on menopur 225. 4 follicles around 14-16mm and nurse thought by EC probably have around 7 to retrieve. She felt with my last AMH showing 26.5, the outcome should have been better and that the starting dose of187 was too low. I understand why they started me on conservative dose (because of PCOS and risk of OHSS) and that it was always going to be trial and error. Nurse provided the option to either continue to EC or abort this cycle and restart it, on higher dose, which is what we've agreed to do. Who knows if the results will be any different but I feel it's worth a shot.
