High Fsh / Low amh / low response to stims

Hi everyone, I was just just looking for some advice/ reassurance. I’m on my second cycle of ivf. My first one i had 3 follicle got 3 eggs, non fertilised with ICSI.

This cycle there is only one follicle responding, docs are saying it’s due to my high fsh/ low amh and age.

Just wondering if anyone is in / been in similar situation?

Also do docs usually adjust dosages after you start stimulation based on growth? As ours is suggesting to just keep it the same.

This happened to me on my nhs round. I have low amh, low egg reserve, endo and adeno. I had 1 follicle with no egg inside. I was then taken in to a counselling room and told I would have to be assessed to see if I would qualify for funding adding to the heartbreak. They also told me they wouldn't change protocol as that's what they do for low amh on nhs.
We then went private and my cycles were:
1 - 6 eggs = 1 blast and 1 embryo
2 - 4 eggs = 1 embryo
3 - 2 eggs = 1 blast and 1 embryo - this was transferred fresh and is now my 2 year old 💙.
Currently pregnant with a frozen embryo from those collected.
4 collections total.
2 transfers both of which were successful.
My private clinic did tailor and adjust the meds as my cycles went along.
Low Amh doesnt mean you cannot get pregnant it just means ivf will be more tricky. Look into clinics that specialise in low amh etc. Wishing you all the luck and strength for this next part ✨

Hi, thank you for sharing, this gives me a little hope. Was there anything in particular you recall the private clinics changing/ altering with the protocol or meds to give such a different result to the nhs?

My first round i had more growth in the follicles but this second round only one follicle has grown which has been very disheartening.

Hey, similar to the above poster I have low amh, had two nhs rounds on max stims, then went private (3 cycle package) and got a lot more eggs on mild stims. Mine (almost) all fertilised but either have poor blast conversion or were transferred at day 3 so I don't know if they made blasts. Consistently get some good day 3s but after day 3 they arrest. Partner v low morphology, used icsi.

  1. NHS, Max Protocol: 3 eggs, all fertilised, one 4CC blast. Failed.
  2. NHS, Max Protocol: 1 egg, fertilised, top grade day 3 transfer. Failed
  3. Private, Mild stims, 5 eggs, 4 fertilised, No blasts
  4. Private, Mild Stims, 4 eggs, fertilised, 1 day top grade three transfer, Failed
  5. Private, Natural Modified (v low stims), 1 egg, fertilised, 4AB blast, 9 week pregnancy so far doing well.

In my NHS rounds I had a good number of responding follicles, I am suspicious about the round I had only 1 collected as there was good estradiol and follicle growth, maybe I ovulated early, even the consultant expressed surprise. But you can easily see that the lower the stims for my eggs, the better. In natural modified you only expect 1/2 eggs, its a focus on quality and letting the body self select and put its enegies into the one or two. I think the stims went from 3/400, to 275, to 150. First two Gonal F, last three Menopur.

Agree with the first poster that a clinic specialising in low AMH will have a different approach to NHS max stims, if thats what they did for you. (Also Im late 30s which doesnt help).
