FET following hysteroscopy

TW: loss

Our first round of IVF blessed us with an amazing two year old, second unfortunately ended in a loss of twins in November - the embryo split and they weren't viable unfortunately. It's been a long and difficult process moving on with the miscarriage with retained tissue finally being removed via a hysteroscopy next week. Has anyone had one of these procedures and moved straight onto a FET? Specifically with Care Fertility? Hoping this is something they sign off as we're really ready to move forward now.

Hi there, sorry for your sad loss.

I’m not with your clinic but I’m due to have a hysteroscopy and my clinic actively want the FET to take place during the following cycle.

So sorry for your loss @daisypizza. I’m with Care and about to start a FET after a hysteroscopy in early January. I have had a full cycle in between but I think that was more having time to sort the prescription etc rather than clinic policy. Mine was an investigative hysteroscopy though so it might be different for you. Good luck.

Thank you both. I'm getting slightly contradictory advice from Care - one nurse said we just need the hysteroscopy report to confirm all clear then we can book in immediately, and another said they prefer to wait six weeks following the procedure (not seen any examples of this being required anywhere else!). Next challenge will be getting the report over to them - NHS trust to trust admin has not been an easy part of this process so far!!

Did you get your findings report on the day @TakeMeOnATrip? I'm worried if I leave there without a print out of something it'll all get held up.

I’m not with Care Fertility but I’ve had hysteroscopies the cycle before both of my FETs (operative with D&C/ polypectomy) - as someone else said above my consultant advised it was best to proceed immediately afterwards. I got a discharge letter and photos on the day and then had to wait about a week for the histology to come back.

I got a discharge letter and photos on the day and then had to wait about a week for the histology to come back.

Thank you 💞 That's good news, hopefully I can also get a discharge letter on the day and finally start moving forward (not sure how I feel about photos in it!!). Hope everything worked out well for you.

I got photos and a verbal update on the day, and a letter a few days later. Care wanted the letter before agreeing my protocol.

Hi just came onto Mumsnet to post something similar! I'm on FET attempt #2 and my lining not doing much despite being on oestrogen pills, patches and pessaries. They've upped my dosage and I'm back for another scan on Friday but I'm pretty sure it's going to be cancelled again unless there's a miracle.

I've had 2 hysteroscopies already to remove adhesions, the result of TFMR surgery, and they're talking now about doing another which I'm pretty devastated by, I really thought that had been resolved and the idea of going through it yet again is a lot to take on.

The doctor I spoke to said it would be 6 weeks of oestrogen after, then off meds for a bleed, then I could start the FET after that, aiming for an embryo transfer to be about 3 months give or take a week or two after the hysteroscopy.

That's good it came in a few days. Was that via post? I'm going to plead with them at my pre-op appointment on Monday to see if they can prioritise it and possibly get a copy on the day 🤞🏼 I know even a few days of delay with post then consultant review could set us back a whole other cycle

Wow what an ordeal you've had @Orangewillow so sorry to hear it. The waiting is the worst part at times isn't it, I really hope things look more optimistic for you on Friday 🤞🏼

Yeah via post. I had the hysteroscopy privately so the admin was maybe a bit slicker than NHS. Hope you get it sorted quickly, the waiting is the worst bit of this whole IVF caper, especially when it’s waiting for admin, it just feels like such a waste of time!

Absolutely, I've done so much chasing and admin throughout this loss and it's taken away from my time to actually grieve it. Miraculously it is the NHS who's fitted me in for the hysteroscopy so I will definitely be all over them for this report then!

I've been in for my pre op today and the doctor doing my consent forms queried how soon we should go back to Care for a transfer as he thought I would 'need time to heal' since they are removing retained product not just looking. I take it you didn't receive this advice from what you said above with your operative hysteroscopies?

@Tiredtoddlerma scan on Friday was much better thanks, lining growing but not at the right level yet, although it all seemed more encouraging! Back again today, keeping everything crossed its still heading in the right direction for a FET soon

That’s right, the advice I had was to proceed immediately (though it was polyps and thickened lining removed in my case). I go to one of the big London clinics and the consultant is a specialist in hysteroscopy, I know two other people who have had a hysteroscopy with him and then had a FET the following cycle so I’m fairly sure it’s standard practice there.

I’m not a medical professional but I’d heard of something called the ‘endometrial scratch’ (where they scratch the lining to hopefully help implantation) and it seemed to me that this would have a similar effect.

Hysteroscopy day at last 🫣 so relieved this is coming to an end but also very nervous, especially for the GA. Wish me luck!

Thank you for your well wishes. It went absolutely fine - all clear now and barely any bleeding and no pain, although the GA has knocked me for six! Did you guys get your period as normal after or can I expect it to be a bit all over the place? Was hoping to give our clinic some rough dates for my next two cycles to plan the FET.

Glad it went well! I’m afraid I was on Norethisterone/ Progynova for mine so can’t help with when to expect a period.

Mine has arrived right on time today! The clinic have asked for another scan on Tuesday too, despite having the report to show everything has resolved. I could do with a break from things up there but nevermind! Did you guys have a progesterone blood test before your FETs too? Do you remember what cycle day this was on?

If I remember correctly it was 3 days after egg collection / starting progesterone, so I guess 2 days before transfer, depending on how the days fall.
