“Lean” PCOS - how to get period back when no cycle atall?

Has anyone managed to get their period back if they have PCOS and no periods atall? A lot of what I read people have irregular length cycles but I have none atall. I’m 32 with a high AMH of around 45.

My BMI is 23, my blood tests show no raised testosterone etc. I have no excess facial hair etc or other typical symptoms. But I have had it confirmed on internal ultrasounds that I have PCOS.

Context - had successful ICSI for DD1 because of partners severe sperm issues (so we had to skip ovulation induction and go straight to that). Neither of us want to go for more IVF for a sibling and no embryos left - it took too much from us emotionally and was a really hard time. So we will be very happy with what we have and forever blessed for this. But if I can get a period back naturally it would give that - extremely small - chance at least of a natural miracle.

Myo inosotil (powder or tablets )? Berbarine? Metformin? Diet? It’s hard to know where to start.

thank you in advance

I haven't had fertility problems but I also have lean PCOS. Sometimes I wonder if I only failed to have fertility problems because I left conception until late -- after 35. Between 15 and 31 I had irregular and few periods. Between 31 and 35 they completely normalised and I think this is common for people with PCOS (as your egg count/AMH naturally falls). I conceived my two kids first/ second time between 36 and 40, and now into my mid 40s still have a regular though shortening (28 days for first time ever!) cycle. So I think you could simply wait. You're still young and your egg quality is probably good. I wouldn't be so pessimistic about having another child.

I’m also lean PCOS but I do have the excess hair and a bit of acne! I take metformin and myo inositol. When I was TTC I was just on metformin, nothing happened until I started taking myo inositol and I then got pregnant. I did sadly miscarry but then started taking aspirin and then got pregnant and had my first child. I’m now 25 weeks with my second thanks to metformin, myo inositol and low dose aspirin.

when I came off the pill I had no periods for a whole year, I found that spironalactone (actually taken for acne and hair symptoms) helped bring my periods back and make them regular. But with spironalactone you have to come off it for 3 months before getting pregnant.

id say start with metformin and myo inositol, and if periods start then start low dose aspirin as well to help with conception.

good luck!
