Urgent! Large follicles CD2 - what should I do?

Hi,

I'm doing a 6 cycle package of natural modified ivf. I have very low AMH levels and am 39. I went in for my CD2 scan today. I haven't taken any medication at this point. I had 3 follicles (which is typical for someone with such low AMH). Two of them were very large - 20 and 22mm - and one was 11mm. My clinic has recommended cancelling this cycle as they seem focused on the 11mm one, which they say may not respond to stimulation because of the other two, which they think must be dysfunctional because of their large size so early in my cycle.

But, my LH is normal and my estrogen is higher than what they'd expect for day 2. This makes me wonder if the large follicles maybe decent, despite coming at a weird stage of cycle. I did have a long cycle this month - 32 days - so I just wonder if my body has got a bit confused. Part of me is tempted to go for an egg collection this week and see what happens, but they think it's risky.

What would you do? Has anyone been in a similar situation? I need to make a decision tonight! Thanks x

Hi @GingerRCD sorry you’re going through this. I’d follow your clinic’s advice, they’re the experts. I’d think it would be unlikely that you’d get a good quality egg from either of those follicles. On one of my cycles I had a large follicle in CD2 which I think my clinic called a follicular cyst. If I remember correctly, they did a blood test to see if it was emitting any hormones and it wasn’t so they said it wouldn’t interfere with my treatment. I continued to egg collection and they aspirated the cyst but from what I remember, didn’t get a usable egg from it. If it had been giving off hormones, they would have abandoned the cycle because the hormones would have interfered with the treatment. If your estrogen is high, maybe that’s what your clinic are thinking. I’m a bit sketchy on the details as it was a while ago so don’t take this as gospel but perhaps worth asking your clinic and seeing if something similar is going on. Good luck with whatever you decide to do.

I had one large follicle very early on, they collected it, it fertilised but didn't get to day 3. As frustrating as it is I'd cancel this cycle and wait until next month.
