Fresh to Frozen transfer success?

DaisyTee · 05/03/2024 09:21

Just wondering if anyone has had failed fresh transfers but gone on to have successful frozen transfers?

My DH and I have had two failed fresh transfers - both double transfers with 4 “top” quality embryos. All have failed to implant.
I’m 41 and DH has low morphology. No other issues.

We now have two “good” quality blastocysts on ice and can’t help but wonder if the timing of the fresh transfer is just too out of sync with my cycle. Which is like clockwork.

Searching for hope that perhaps the Frozen transfers will be more in sync with my natural cycle and work. Anyone out there felt like this and gone on to have success with Frozen transfers?

@DaisyTee I'm about to go into a FET, not a fresh before as we did embryo banking. Have you had any other investigations done, such Hysteroscopy or auto immunity tests?

DaisyTee · 05/03/2024 09:57

Best of luck for your FET!

I asked about other investigations after first failed round but was encouraged to just go again as there was nothing on initial scans or blood tests to indicate anything wrong.
My clinic seem to offer sweet f.a. on that front. They don’t offer Hysteroscopy but do HyCoSy so I am going to push for that. I’ll have to look into auto immunity tests elsewhere. Did you have these tests done before?

@DaisyTee apologies in advance for the long message but want to give you all info/context from my end! I've had miscarriage tests after 1 chemical pregnancy summer last year (natural pregnancy so likely a chromosome issue perhaps but you can never be 100%). I was advised its only usually needed after 2 miscarriages but I wanted it done (plus we're private so cost was on us so Dr left it to us to decide)- through this some things got ruled out (sticky blood, lupus and things like that) but I found I out I had thyroid antibodies which can cause miscarriages so I'm on steroids; Prednisolone for the FET to hopefully counteract any reaction (studies are mixed if it works but worth a shot).

I also had a Hysteroscopy as I got a polyp through the egg collections (can be a side effect with added hormones, and I had almost back to back cycles). Through this the polyp was removed but I also had the lining smoothed over as I had adhesions (unclear where they were from, Dr asked if I ever had an infection, but I cannot recall I have but I'm 38 so could be an age thing perhaps), a biopsy was also taken which was clear and my cervix was also too tight to have been able to perform a embryo transfer with the tools needed so that was opened a bit so FET can take place. So all in all Hysteroscopy was well worth me having.

We've also had Karyotype testing before starting egg collections to rule out genetic conditions which came back clear and I have had thyroid testing to keep a check on my thyroid levels (needs to be under 2.5).

There are other tests but right now will just roll the dice and see what happens.

I have elected against PGTA testing mainly because our embryos are already frozen and I have been advised we could lose an embryo through the freeze/thaw/test/freeze/thaw process. Plus a part of me feels its an evolving procedure...bit nervous about discarding embryos when you read they could 'self correct' inside.

Anyway sorry for the long post but that's my experience! I think it's worth ruling out what you can before transfers but I think that mainly because I have low AMH and want to give the embryos we've got the best chance as my numbers over time will inevitably decline further and age is not really on my side

DaisyTee · 05/03/2024 10:38

Thanks so much. I think I just need to push on what tests can be done whether they can be carried out at the clinic or not as it’s difficult to know what to ask for.

My thyroid is in normal range. AMH is reasonable for my age.
I’ll start with diagnostic Hysteroscopy / HyCoSy and work from there I guess.

Thank you!

I had 3 consecutive natural MCs then I had two failed fresh transfers, my first FET was a success and I'm now 36+2. I am now 41.

LittleDoveLove · 06/03/2024 10:28

Will add the first two failed fresh transfers were 3BC, the FET was a 5AB. I also checked my lining was thick enough for implantation, I wanted optimal not just acceptable x

DaisyTee · 06/03/2024 10:30

Aw @LittleDoveLove thank you so much for sharing that, gives so much hope! Do you think it was down to the timing of the frozen transfer - was it more in line with your natural cycle?

@LittleDoveLove , thank you for sharing your experience! and congrats on your first FET success! Would you mind to share how did you feel after the transfer the day of and the days following leading up to your BPF? what was the process like? did you have a natural cycle transfer?

TumbleweedAgain · 17/03/2024 11:34

@DaisyTee , I have had 3 failed fresh transfers, and the only BFPs I ve ever had were from the FETs I had (6 of those). I think it is more about the quality of the embryos rather than timing. The grading used by the clinics only shows the “external” quality but says nothing about their genetics.

Sunflower360 · 17/03/2024 11:51

@TumbleweedAgain if you don't mind me asking did your FET work? I've just had a failed FET and am planning my 2nd one now

Imisscoffee2021 · 17/03/2024 11:58

Failed top graded blast fresh, 2 momths later successful first frozen. Transfer on a Thursday and positive test on the Tuesday!

Sunflower360 · 17/03/2024 12:48

@Imisscoffee2021 if you dont mind me asking what grade was your FET and what was your age when embryo was made? I'm 38 have 7 embryos left (6 x blastocysts and 1 x day 3) so considering if the next couple of FETs don't work whether it's worth doing another collection to bank more if we can between transfers

Imisscoffee2021 · 17/03/2024 13:05

@DaisyTee I was with Care and they don't do the 5AA etc, they grade from 1 to 10. They never told me the exact grades, just said we had top and good grade embryos. We had 6 from 7 eggs, so I assume the FET was probably top grade as the fresh was too. I was 34 when we did the round and got pregnant so have 4 still frozen. You have a great amount to try, fingers crossed! For me what helped was lubion injections, the pessaries just seemed to keep me at 35ish for progesterone and while that's above the clinics threshold it's not that high so I paid for lubion. Also took high dose vit D as was advised it can aid implantation. I felt much more myself on the FET too, not so funny of hormones compared with the fresh.

Sunflower360 · 17/03/2024 13:23

@Imisscoffee2021 thanks! What dose vit D were you taking? I'm hoping my progesterone levels are ok but will ask to have it tested. It was around 60 when I had a natural chemical last year which was bolstered with progesterone pessaries as I started spotting straight after implantation until AF arrived, so hopefully that's good

TumbleweedAgain · 17/03/2024 14:25

@Sunflower360 , yes, I am now pregnant with my 2nd IVF baby. 1st round didn’t work-BFN with the only embryo. 3BFNs, 1 chemical and 1 baby from the 2nd round, and 2 BFNs and 1 BFP from the third round.

LittleDoveLove · 17/03/2024 17:12

@Peonies19 Thank you 😊. I felt fine after it, I actually went to a kids party with my son to take my mind off it then had a little rest when I got home.
I tested early (5dp5dt) with my FET as I had signs that I had before after a positive test. Mainly pretty gassy is the one for me. X

LittleDoveLove · 17/03/2024 17:15

@Peonies19 @DaisyTee I had a modified natural FET. So I actually had sildenafil (viagra) so help thicken my lining (I asked for that - it made me absolutely awful). Stopped that when I got my smiley. Then I had oestrogen after I got my smiley and then Progesterone started a couple of days before the transfer: my transfer was 6 days after the smiley - I ovulated naturally (no trigger) X
