IVF recurrent miscarriage whilst on blood thinners and steroids

Hi. I was wondering if anyone has had something similar.

About us:
Sam sex couple. My wife is in her 20s, had AMH etc all good and lots of follicles (40ish) (PCO but no not PCOS). Slightly high Thryoid levels but am on levothyroxin for this so level is within normal range.

Journey:

  • We had two failed IUIs.
  • We then did IVF where 31 eggs were collected but only 1 made it to day 5, 2 more were taken to day 6 and then were good enough to freeze. The 1 was transferred and ended in chemical miscarriage at 1 day after positive test.
  • Then we did a frozen transfer which lasted 5 and a half weeks ending in a miscarriage.
  • Followed by another frozen transfer ending at 4 weeks again but I also had COVID.
  • We have just done another IVF cycle where 23 were collected and only 1 made it to day 5 again.

Tests done:
We are using donor sperm, they are not a carrier of anything and kayrotype normal. My wife’s Karyotyping was also normal. She has Thryoid antibodies and high NK cells.
Haven’t done PGT testing due to only having 1 embryo each time but we have recently sent some miscarriage tissue to NHS to be tested however.
Had NHS 3D scan of uterus and all is fine no abnormalities.

Medication that my wife is on:
⁃ Prednisone and Intralipids for NK cells
⁃ also on asprine and clexane for blood
⁃ Lubion injections and pessaries for progesterone.

We are lost for what the issue might be.
Has anyone had recurrent miscarriages on all these medications as well? Anyone found the cause?

We know PGT testing will probably be raised here quite a bit. Our thoughts on this are; we know the embryo could be abnormal either because of one person being a carrier of errors in chromosomes or random abnormalities. As we know my wife and donor aren’t carriers that leaves it down to random abnormalities. Misscariage has happened 4 times now and she is young, fit and food conscious so wouldn’t understand why all 4 have randomly been abnormal.

Thanks

There's a Facebook group called reproductive immunology support, you'll likely find more people with a similar situation there.

I would worry about the fact the clinic is getting you so few embryos - I would expect a lot more in my 20s! Perhaps a clinic quality issue?

can also ask about antibiotics before transfer to treat any underlying issues.
