IVF large numbers collected but only 1 embryo

My wife has had two IVF cycles. She has PCO but not PCOS (so she has lots of follicles but no insulin resistance).

Cycle 1 ended with 31 collected and only 1 embryo on day 5, then two more on day 6 that were good enough to freeze (A or B grade).

Cycle 2 ended with 23 collected and only 1 embryo on day 5.

has anyone had anything similar with this much of a drop off? has anyone concludes why and whether its due to poor quality/abnormal?

Havent done PGT testing of any of them. We have had 4 miscarriages with them all. Same sex couple using donor, donor has been screened. My wife has had Karyotyping, blood clotting, internal scans, NK, progesterone levels, Thryoid and antibodies all tested.

I had my egg collection Sunday and had 21 collected, just had my day 5 call and only 1 has made it to a poor quality blast, 4CC
Feels like such a blow
