High prolactin, pelvic pain?

I'm wondering if anyone had experience with these symptoms or can advice.

38ttc almost one year. Started fertility investigations early, AMH 10, tubes clear, but they did identify a polyp which I had removed via hysteroscopy three months ago.

In post op bloods my gynae found high prolactin (above 600) and I'm now waiting for referral to endocrinologist and MRI. Hoping I can get some medication to fix this if it's still reading high at that point.

For the last few months I've been getting pain in the left side (pelvic pain) mid cycle with no particular link to ovulation. I'm CD16 today and have has this dull ache for about 4 days now, left side only, not sharp but more nagging / heavy feeling.

On top of that I feel terrible, lethargic and low level depressed. Thyroid and oestrogen were normal in bloods but I'm wondering now if these can symptomatic of the prolactin or something else? Could this be endo, PCOS?

I've booked in for full hormone panel including Day 3 and 21 in the new year, but I'm sick waiting for this referral and wondering if I should go back to gynae with this new symptom in the meantime?

@wonderingwonderingwondering I have high prolactin due to microprolactinoma and that isn't a symptom that has ever been mentioned to me. They usually ask about headaches, irregular/absent/light periods, leaking breasts and vision problems. I do however also suffer from pelvic pain but that is due to endometriosis.

I'd go back to your gynecologist to investigate as endometriosis isn't usually picked up on scans unless there are cysts.

Hi @Whatamess12, thanks for sharing.

I couldn't find any information on pelvic pain being related to high prolactin and was worried about endometriosis as it doesn't feel like it's just normal ovulation pain.

I've called my gynae's office about it and am now just waiting on a call back. The waiting waiting waiting is the worst part of all of this - it's been weeks since the high prolactin was identified and no sign of my referral yet.

Would you mind sharing how you managed to get diagnosed with endo? I've had pelvic scans previously, the last one revealed a polyp which I had removed via hysteroscopy, but no mention of endometriosis etc.

@wonderingwonderingwondering yes I had a "chocolate cyst" on my ovary which does show up on a scan and as a result of that had a laraposcopy.

Hi @wonderingwonderingwondering I've just discovered this thread and wondered if you'd have any answers? I've had all my bloods (day 3 and day 21) done and the only thing that is elevated (just above 600) is my prolactin. I've had severe post ovulation pain for the last 6 cycles, I had a laparoscopy/hysterycopy and Hycosy for suspected endo but it all came back clear. The pain is quite severe and can be any time in my cycle so I too am wondering if it could be down to the elevated prolactin as thats the only thing that has come back high. im 41 and ttc #1 for over a year. I had a mmc in March and the pain seems to be worse since then but the consultant said it wasnt from the surgical management. its very frustrating as i feel like something is wrong but i dont know what it is 😣

Hi @Sez281, sorry that you've been experiencing similar, I know how worrying it is. Also sorry to hear about your MMC :-(

I've still not gotten any answers, have fallen victim to Christmas scheduling and haven't heard back from my gynae yet. However I'm booked in for bloods and an MRI to check for a putuitary tumour in January. Pelvic pain has mostly persisted through AF which came about a week ago, albeit it's definitely less noticeable now. It started more than a week before ovulation and has lasted more than three weeks now so don't know what to think of it. I've stopped googling as I was driving myself demented, although from what i was reading I had a strong suspicion it could be something like an ovarian cyst if not related to my hormones / elevated prolactin.

I'll update if I get to the root of it, as I know how frustrating it is to not get any answers for this kind of thing.

Thanks for replying @wonderingwonderingwondering I also have an appointment early this month with the consultant (first meeting since the surgery) I'm hoping she might be able to shed some light on the pain. My GP has also said that I'll have to have repeat bloods and possibly an MRI in relation to the high prolactin but to be honest I'm not to worried as it was only slightly elevated and I've heard it can be caused by stress (which let's face it TTC is!) hopefully we both get some answers soon. I'll keep you updated too!

hi @wonderingwonderingwondering I hope you are well. Please, I hope you dont mind giving an update on what happened. I honestly did not expect to see this post as i have for a while had issues with left sided pain similar to what u described but got worst more recently and worried it might be endo. I have also not been successful as yet with conceiving. I happened upon this post cause i have high prolactin and due an mri scan to check for prolactinoma but seeing that you have other similar symptoms to mine was so coincidental. Thanks for sharing.

Hi there @carsty , sorry you're experiencing pain and no doubt a lot of stress.

My brain MRI came back negative for a benign tumour, and subsequent tests showed that my prolactin came down from 600+ to 400, which also coincided with quitting a very stressful job so likely the spike could've been stress related.

I also did a pelvic MRI, which revealed adhesions on the ovary with the pain (likely endo) s well as suspected adeno, and am currently on the waiting list for a lap with an endo specialist which hopefully will happen within about 6 weeks. After that it's straight to IVF as still no BFP after 18+ months ttc, we made the call to do the lap first to give ourselves the best chance with that.
