Progesterone 27.4nmol/l . advice

Hello, I had the results come back from my progesterone test, taken 7 days after my period .the results were 27.4nmol/l . ( I paid for this via medichecks)
The doctors comments were that this falls in the grey area and can't confirm if ive definitely ovulated.
I'm just looking some advice from anyone who has been through similar or can understand it
I'm currently on clomid ovulation induction medication. Im on my third cycle.
On my first cycle my day 5 progesterone with nhs was 32.5
Nmol/l which confirmed ovulation.
Second cycle I did not have levels checked.

It's absolutely Impossible get my nhs gynae consultant to contact me back . Do you think these results are concerning at all and should I really push for an appointment with him?
In the event I did ovulate, is 27.nmol/l enough to support implantation?

Hi @MK85 did you mean it was taken 7 days after ovulation? As I don't know why they would test 7 days after period. Ideally they need to test 6-8 days after 'peak' on a ovulation test, do you track ovulation?

I was always told anything over 30 during the above timescales is normal.

wtawtaw · 31/07/2023 13:32

Sorry what I meant was your peak progesterone is 7 days after ovulation.

MK85 · 31/07/2023 13:38

Sorry yes,my mistake I meant 7 days after ovulation 🙈

Yes I track ovulation with opks and temp

Ah ok, did you track ovulation or did they just assume you ovulated on day 14? (Which is why they suggest day 21 bloods but not everyone ovulates on the same day)

Yes I track ovulation with opks and temp

Hmmm ok, temping is usually a good indicator, what do your charts look like?

MK85 · 31/07/2023 13:59

@wtawtaw this is my temp chart

@MK85 the temp shift would suggest you've ovulated but they are still on the low side which matches to your test of low progesterone. Obviously I'm no doctor so just going by the amount of research I've done on my own infertility for the last 3.5 years. Do you get spotting before period, a short luteal phase? If so then maybe you need progesterone support too. Low temps can also indicate thyroid imbalances too, have you had that checked?

wtawtaw · 31/07/2023 14:17

Either way I would definitely push for more investigations/various blood tests if this is all they've given you!

MK85 · 31/07/2023 14:49

@wtawtaw thanks.
I have brought this upto my consutant previously regarding low progesterone as its something thats been on my mind all year but he seems extremely uninterested and told me everything is fine as my first month on OI was 30.5nmol/l
My luteal phase over the last few months is around 17days.
( although I did have a very short luteal a few months ago of 9 days. I used to spot in the past but havent all year)
I've already had one chemical pregnancy in december .
I've 3 more months of medicated ovulation induction to complete before ivf. It does also concern me that I may be wasting these months if I'm not definitely ovulating or have low progesterone..I've already wasted years waiting for them to get the ball rolling and I'll be 38 this year ttc #1
I'd just wanted some confirmation regarding my recent progesterone results if I should keep pushing at him as I was always told it needed to be above 30 to confirm I'm ovulating, otherwise dose would need to be increased if its not working.
Dealing with him is just a real pain in the ass. He told me they wouldn't look at my progesterone levels unless I've had more than 3 pregnancy loses.
I'm considering if it might be worthwhile to go private to get their opinion on progesterone.
I had a full hormone profile blood test 12 months ago with nhs, including thyroid.
Nothing has been repeated since other than progesterone, amh and prolactin in feb so they should have enough information

Do you agree my temp chart seems to accurately indicate ovulation as per interpretation on cd18 or could I have just ovulated a few days later

@MK85 I would say you ovulated either on day 18 or 21 as I have previously read it dips before ov. What day was your test on?
I would definitely keep pushing or go private - as you say you don't want to waste any more time and don't want to end up regretting not doing anything sooner. I too had to go private for my endo surgery as NHS just put me down as painful portions and I couldn't possibly have endo as I had regular cycles. Turned out I had stage 3 endo!
Do what's best for you but I certainly wouldn't want to wait until I had 3 miscarriages - how they can they say that is disgusting, who wants to go through all that trauma before being treated seriously 😢

wtawtaw · 31/07/2023 15:12

Painful periods that was supposed to read 🙃

MK85 · 31/07/2023 15:49

@wtawtaw did you have laparoscopy? How much did that cost you privately? When I was last with the consultant he mentioned to me that he thinks I could have endometriosis but has decided against surgery to investigate 😒

@MK85 yeah I did, it was just over £6,000 but hopefully worth it if it gives us a little one! It's weird as when I had my consultant he put a stage on it just by me telling him my symptoms - and he was right!
If you do go down that route, i would opt for excision rather than burning as with burning it can grow back so much faster. My pain improvement is incredible! With NHS they are likely to burn it as it's the cheaper option. It's worth looking at the Nancy nook website/Facebook group for a specialist excision surgeon too.

Do you have endo symptoms too?
