TTC: Day 3 and 21 blood test results

Hello,

We have one Dd who is is nearly 4 years. Fortunate in that it took around 6 months TTC with Dd. We have been TTC again for the last 24 months. We did conceive in Sep 21 but sadly resulted in a MMC diagnosed at 11 weeks. I am 35, OH 39. Started the process via GP for exploring fertility in Nov last year due to this and also irregular cycles (24 days length, to sometimes 44 days).

Had a blood test in December not for a specific day but happened to be my day 4 of my cycle. The blood test was testing Vit D which was low 34 nmol, prolactin 413 miu/L, oestradiol 98 pmol/L, FSH 3.9 IU/L, LH 1.7 IU/L, Serum testosterone 1.4 nmol/L, serum sex hormone binding glob 28 nmol/L, free androgen index 5 %, TSH 2.06 miu/L, B12 261 ng/l, folate less than 2.2.

Following this has 2 repeat ultrasounds of ovaries and pelvis (repeat as unclear scans) and so happened to be day 21 for one and day 14 of cycle for them in each different cycles. Both unfortunately were unclear but one showed slightly enlarged volume, the second both ovaries slightly enlarged volume. As they were unclear they weren’t able to count follicles.

I had a further day 3 blood test in March was actually on day 3); my periods have begun to become more regular at every 28/ 29 days. Day 3 bloods:

FSH 4.2 iu/L
LH 1.1 iu/L
B12 242 ng/l
folate less than 2.2

Then day 21 bloods (on actual day 21 of same cycle)
progesterone less than 1.6 nmol/L is all it says…

I have been advised to go contact a sexual health clinic to get that done first, partner to contact GP and arrange semen sample, and then they will refer to fertility service.

I do take folic so GP is writing script for higher strength folic. Im supplementing with Vit d.

Apologies for the long post, unfortunately have had only one face to face app and that was before these results, was given these over phone but managed to get a print out from the surgery today. The doctor on the phone suggested her interpretation was that it’s unlikely I ovulated this latest cycle due to progesterone level. Gp didn’t suggest anymore on whether I could have PCOS. I do get terrible acne on back and at times face, hairy chin, tummy, sides of face, nipples etc, weight gain and mood swings (this has been the case for as long as I remember).

i have done Googling but am wondering if I do have PCOS but unfortunately with being unable to count follicles probably won’t get a diagnosis and a I am also wondering if perimenopause/ low reserve. From these results can anyone with any fertility testing experience be able to point me in direction to interpret them a bit more for where I am in cycle on any advice?

I can answer only for prolactin oestradiol fsh lh tsh which are all good for day 4

FSH 4.2 iu/L
LH 1.1 iu/L
B12 242 ng/l
folate less than 2.2

Of these lh is low fsh ok the other I don't know

Then day 21 bloods (on actual day 21 of same cycle)
progesterone less than 1.6 nmol/L is all it says…

When this happen it means you didn't ovulate that cycle

This is everything I can help you with... anyway with your results, along with any test, you should have the range to analize, this is how you can understand if that is low or high

It seems you have pcos from the symptoms, try pure inositol helps with ovulation and drink grapefruit 1-2 glass per day from when your pariod ends and until one day after ovulation.

@KoraPersefone thank you. Yes from my research the progesterone seems very low and indicates I didn’t ovulate in that cycle (possibly all my cycles) with lack of being able to conceive. Iv tried researching LH but don’t think there’s anything that can increase that?

There is always something that can be done even to level up lh but your gp must do that, you can't do anything. If you don't ovulate i would suggest to take inositol that helps, no need to go to gp for that but even if you ovulate but your progesterone is low, even if the egg is fertilised you could not be able to keep the pregnancy anyway, so you must check your level during an ovulatory cycle to be sure you don't need help with that.

thanks @KoraPersefone, whilst I’m waiting on fertility referral input, do you think it’s worth me approaching my doctors requesting another day 21 blood test for progesterone levels? I can try I guess. As the day 21 was done this cycle and as of yet I haven’t started my period (I’m currently day 30 of my cycle at the minute). I will get some inositol see if that helps :)

Can I ask how your journey is/ where you are?

Yes you should redo that to see if you ovulate and to check that there is enough progesterone to support a pregnancy.

Where am I at? Well I'm pretty much heart broken at the momento, today all I did was crying but, anyway, I'll get over it somehow, don't want to invade your thread with my story.

15 months ttc low ovaric reserve 35yo hoping to start ivf in the next 4 months (yesterday I was put on the list). There is no much hope for us unfortunately, if ivf will not work, our journey will end there.
