Time before egg collection (PCOS)

Hi there,

Looking to understand the day of people's cycle they had their egg collection as I am due to start stims but need to plan it round work/being out the country. Looking for a rough idea to see how often people go over 14days. I have a PCOS as well with AMH 68, so thinking that might mean shorter cycle. I have already tried OI with letrozole & menopur but didn't ovulate.

Please let me know, so I can get a rough idea - thank you!

I was a slower responder so had 12 days of stims which meant EC was on day 17 of my cycle.

I had my baseline scan on cycle day 2, started stims day 3, for 12 days which takes us to day 15, did my trigger that night and then ready for egg collection 36 hours later which was day 17.

@10degreessunny it's so tricky, and everyone is so different. Leaving the country must add an extra layer of complication! Good luck. When do you start?

Yeah, and then work commitments which I know aren't as important but can't help but want to plan around them. I'm hoping to start stims on the 12th May, and have EC by the 30th May as go away on the 31st.. that's 19 days so I feel that does give me time.. how are you getting on? Do you have PCOS? X

Hi.

I have PCOS and a similar AMH to you (64).

I have done two IVF cycles and stimmed for 9 days each time. Egg collection on day 10 and fresh egg transfer on day 15.

So if I was starting stims on 12th May, I would expect:

Baseline scan 10th May
Start stims 12th May
A couple of scans/blood tests around 16th and 18th.
Final scan on 21st + trigger
Egg collection 23rd
Fresh transfer 28th or 29th

So you are cutting it fine if you Baseline scan is actually on the 12th (although you might start stims that day, it depends on the schedule set by the clinic).

The other variable, is whether you are having a fresh transfer, as the cycle is shorter of you don't. You may not get any blasts or you may freeze all the blasts you make. Or, I guess you may do a day three transfer.

Anyway, above is a rough guide for high responders on short cycles like me.

Thanks so much for sharing your experience. I am doing a frozen transfer in a few months so this month will just be egg collection, so soundd like it could be a lot sooner if you were day 10 for EC. They're making me do frozen because they're worried about OHSS. Did you have any issues with this? Thanks so much for spending the time to share this info.

So they were cautious with me as I had pcos and an AMH of 74. I was stimming for 14 days!! Not much was going on by day 5 so they increased the drugs.

Meant to add I had a fresh transfer and got pretty bad OHSS. It was rough.

I got a little OHSS on my 2nd round, but it passed quickly. I have been triggering with a non- HCG trigger which reduces the risk.

My next step is to have Ovarian Drilling followed by a round of IVF, as we are hoping to improve my egg quality.

@Kay00, ah okay, good luck with the ovarian drilling, I don't know much about that, is it good for PCOS?

@10degreessunny yes, I think Ovarian Drilling is the traditional (old fashioned) treatment for PCOS. It is exactly what it says on the tin: they drill holes in your ovaries. It's meant to help release liquid and reduce the hormones which contribute to bad egg quality.

My consultant said she had a patient who had an AMH in of about 120 (!!) And after several failed rounds of IVF they tried OD followed by IVF... And from that round she got pregnant. The same thing happened when this lady wanted her 2nd child... A few failed rounds of IVF, and success only followed her round with OD.

Im not holding out a lot of hope, but it might give me a better chance to produce more blasts.

@Kay00, ah okay, good luck with the ovarian drilling, I don't know much about that, is it good for PCOS?

Egg collection was ok in itself and they got 17 eggs. 15 viable for ICSI and 8 fertilised . The drop off rate was massive with two blasts on day 5 with only one that was good quality.
They blastocyst is now a cheeky 16 month old little boy!! So I'd say it was pretty successful. My advice would be don't get bogged down on the numbers because even if you have lots of embryos , they need to implant. It only takes one. I appreciate we were very lucky.
I did get OHSS which was made worse because I got pregnant from the transfer. I was in a lot of pain. It settled down when I was 7 weeks pregnant.

@decor340 that's very positive news to know - congratulations. I am really intrigued by how it will turn out for me with the egg collection and thats even before transfer. Transfer seems a long way off!
