AFC scan - help please!

Hi all,

Long story short: I am almost 37, I have one almost 7 year old child (born in 2016) who I conceived second month of trying. Uncomplicated pregnancy but assisted birth and turns out he had undiagnosed IUGR and a small, black placenta that was dying. But that's by the by, he's very happy and healthy now.

We began trying to #2 in March 2019 and had three chemicals, in May 2019, August 2019 and January 2020. Was referred to the recurrent miscarriage clinic in London but didn't get there until July 2021 due to covid (and not living in the UK so travel was not possible at that time). All tests normal. Husband semen analysis normal.

Haven't managed to get pregnant at all since my third loss in January 2020.

I have had (in no particular order)

  • Numerous scans
  • HSG
  • clomid
  • cyclogest
  • all the blood tests you can imagine

Official diagnosis of unexplained secondary infertility.

We have now decided to try with some proper fertility treatment and have contacted a fertility clinic in the UK. They have asked for another semen analysis, an AMH blood test and an AFC scan.

I had my scan yesterday on cycle day 19 and I attach the report to this post. I had 7 follicles in my left ovary and 9 in my right. He said my follicles are tiny and it looks like I haven't ovulated at all this month. Is this right? My periods are regular (currently 24-27 days) and in previous scans I've had nice big follicles and ovulation has never been an issue for me. I'm obviously aware of my age and wondering if this is the beginning of the end for me?

i read that follicle count and AMH means nothing without the other, so until you have your AMH result back, it’s definitely not the end of the road.

you have a good number of follicles, and perhaps this cycle is an odd one, especially if previous scans have shown bigger follicles.

Perhaps one of your follicles is still growing and you will ovulate later than usual. It happens, and for no reason.

try not to go down the Google rabbit hole and wait for your AMH to get a clearer picture.
