IVF: 1 or 2 embryos?

Hi everyone.
I need some advice please!

I am 34 with but slightly low AMH for my age group. DH is 36 with low morphology and motility.
First IVF/ICSI round on NHS was:
-15 eggs
-10 fertilised

  • 1 5AB fresh cycle ended in BFN
  • 1 4BB FET also ended in BFN
(No more embryo made it, we did not do testing)

We changed clinic and from this current IVF/ICSI:
-27 eggs
-18 fertilised

  • day 5: 1 4AB, 1 4BB & 1 4BC
  • day 6: 2 4BB, 1 4CB
  • day 7: 1 4CB
(Again, no testing cause we cannot afford)

Now, our Fresh ET with Day 5 4AB in May ended up in an early miscarriage at 6weeks...

We will prepare for a FET in June/July and we are considering the option of transferring the two remaining 5 day embryos 4BB and 4BC.

We are very undecided, so please share stories and opinions if you can :)

Given you've had 3 previous transfers I think going for 2 is reasonable as long as you are prepared for twins . I've had 2 cyles and had 2 embryos transfered both times. Second cycle bfp with a singleton. That being said I'm over 40 so double transfers are pretty standard for my age bracket.

I wouldn’t personally, twin pregnancies can be high risk for both mother and babies. Also if there’s an undiagnosed problem with the uterine environment that’s caused the failures so far then you’ve potentially wasted two embryos.

I have twins from a double transfer but know plenty of triplets from a double too so only do it if you are prepared for multiples in every way

I love it and it's amazing being a twin mum. But pregnancy was hard and despite being relatively young fit and healthy we all nearly died during birth and it's likely one of my twins will have long term health issues which can only be a result of being a twin

But I would do it all again in a heartbeat

I wouldn't get too hung up on transferring the 2 days 5s together - my twins are a day 3 thawed and cultured to day 5 and a day 6 which was frozen so it's more about compatibility of the two you transfer rather than the day they made blastocyst

Something we’ve been wondering too. We asked our consultant who was able to give us lots of stats on women they’ve treated the same age as me and they did said I had slightly more chance of success with two single transfers than one double, plus, im really not mentally prepared for twins so that’s just a personal choice too. Might be worth asking your consultant too.

We are aware of the serious risks in twin pregnancy and still evaluating if it's for us (seems such a remote option that both embryos would stick , but we know there is chance of it). We are both happy although with the idea of having twins if both embryos stick.

I am not sure what other tests we should go for beforehand, as both NHS and IVF clinic seemed to think that failed implantation and early miscarriage were probably cause by embryos quality and abnormalities. Any suggestions of what may have helped you?
This cycle I was on high pregesterone (suppositories and injections), steroids, blood thinners, baby aspirin etc so
I'll speak with my consultant again on Monday, so see if she suggested something.

Financial side does also play a huge role for us, as each transfer cost a lot, especially after a private IVF fresh cycle, so on that we also have to be realistic on what we can afford.

Hi guys, I thought I'll update you.
After the CP, Dr realised I had hydrosalpinx in both tubes, that had a negative impact on uterine environment for embryo development. I had surgery to disconnect them.

Ans we decided to transfer two embryos in October.🤞🏽🤞🏽

Hi guys, updating the forum:

I am in the TWW (3dp5dt) after transferring the two remaining day5 embryos (4BB & 4BC).

it was an hard decision, but both embryologist and Dr were happy with, considering two missed implantations, one miscarriage and the financial aspect, which is an issue for us.

will update on results!

Just on here to say I’m also in the TWW after transferring a 4ab and a 3bc embryo… it was recommended as I had one failed transfer of a fresh top grade embryo previously and the 3bc was borderline for freezing, so made sense to use both rather than potentially losing one (I’m a bit older than you at 38, though). Very hard to get head around what’s the right decision on these questions but feel comfortable so far with going ahead with the double transfer.

Fingers crossed for you - the TWW is agony, isn’t it?!

Yes, TWW is pure hell. Fighting hard against the urge of testing. I am on my 5dp5dt so I would be silly from me, but so tempting!

How are you getting along?

Thanks, @morganablue! I’m doing ok. I’m 8dp5dt so getting anxious about testing now! I’m waiting to Day 12 (Monday) as asked - probably because I don’t want to see it if it is bad news! Gotta keep the dream alive 😂

I’ve had the odd twinge and felt a bit tired occasionally - which both could just be the progesterone - so not feeling many symptoms. Trying not to torture myself too much!

@Yazoop how are you doing lovely?

I tested today at 8dp5dt (it would have been at 1pm) at 6am as I need to pee so badly!
With a very very guarded heart we are cautiously celebrating a BFP, but we know is a long road ahead and we hope for a good Beta on Monday!
Lines also looked good and much better than our CP last cycle.

Sadly it is a BFN for me. We’re disappointed but going to take some time to heal and try again. Onwards…
