Help with form for fertility MOT

Hello can anyone help me with forms I'm filling for fertility MOT...In a section of pregnancy information it's asking date of infertility start (mm/yyyy). Is this the date since we have been trying to conceive? I'm confused please if anyone has come across this and can help, I'd appreciate it. Thanks

I think in the absence of them providing any more information, to put the date you started trying to conceive. As part of this assessment I should think you will see a specialist who will take your history, so you can explain the specifics then.
