Baseline scan only 2/3 follicles (DOR) go ahead or wait?

Just that basically.. I have dimished ovarian reserve and had my baseline scan for my second cycle of egg freezing (first cycle 4 follicles flushed and no eggs :( )

On other scans I have had 3/4 follicles on my left ovary and 1 on my right. My right is notoriously difficult to even view on a scan let alone reach at egg collection though so I've kind of ruled that out.

Today, day 1 baseline scan (as opposed to usually day 2 or 3 due to weekend) I only had 2 follicles left and maybe 1 on right.

Should I go ahead anyway or wait another month or so? I guess no guarantees of any more follicles next cycle..

So gutted as I've been on 600mg coenzyme q10 for 3 months since last failed EC and I'd hoped it might improve chances! Wondering if the fact it's a day or so early make a difference? I guess with DOR it might just be the case that I'm rapidly losing fertility (amh 0.8pmol and am age 34).

I did have a few other follicles grow from stims last time but the only follicles of any decent size were the ones I started with on my baseline scans so I'm thinking this means best case scenarios of 2 eggs.. sigh..

@Rosiestraws I think 2/3 is pretty good for 0.8pmol. I started with 5 on one cycle but ended up with 7 so a few more might pop up...

To Be honest with DOR I'd go ahead with collection x

Thanks for the replies.

@MabelG don't suppose you know how many of your extras that popped up were any decent size for EC? I had 4 on left and 1 on right before and I think two more popped up from stims but then I don't think they were the ones that got to a decent size as it was always 4/5 that were a decent size throughout my last stims and 2 that never got that big..so I assumed it was whatever I started with that actually responded well to sims and the other 2 were no good?

@ivfgottwins just to clarify I haven't started stimming yet.. trying to decide if I should go ahead with the cycle and start stims.. the left has usually had 3 or 4 follicles previously on scans so this is less than usual with only 2 :(

@Rosiestraws they were all a good size for EC although one of them was not accessible unfortunately. It is hard to know what to do for the best. How many cycles have you done? My usual AFC was 2 (AMH 1.0) so that was just one particularly fortunate month.

Rosiestraws · 19/06/2021 09:51

@MabelG Yes same for my right ovary...pretty sure it's not going to accessible so I'm just ignoring that one..

I've done one cycle before, this is cycle 2 and I have a 3 cycle package.

Im thinking now maybe I'll start stims tonight as I'm supposed to and then see how they're going on my scan on Thurs and then make a decision if I'm going to cancel or go ahead? I'm pretty sure there's no issue with stopping before egg collection (although any thoughts/confirmation gratefully received here!) I wont have paid as much as a full cycle as I'll just have to pay for my 2 scans and the medications I've used until then....? 🤔

@Rosiestraws yes that is a good plan, I had a cycle I had to abandon due to poor response which was annoying but the cost was no where near losing a whole cycle. Best of luck!
