Which documents do I need to send to IVF ?

I have to fill in a form and send it back and My letter says , please can you provide two forms of ID , one photographic and one proof of address for both of you ..
I understand I will include a original letter for address ?that’s fine , but do I have to send my original passport or driving licence ? Or can I take a picture of them and print it out ? I am so confused ! Thank you

Is this for your fertility clinic? I think it will depend on the clinic. I don’t remember what I did, but I’m pretty sure everything was online and don’t remember having to take anything in.
