Consent Forms

Hi, does anyone know if you need to complete new consent forms every time you start a new cycle or do they last for a period of time - eg. I thought they maybe lasted 3 months so if you started 2 cycles in that timeframe you didn’t need new consents - or have I made that up?

Hi you don't need to do them for every round. They simply expire after a certain period of time. I'm afraid I can't remember how long now. I think it's either 6 or 12 months. Your clinic will be able to confirm.

I always get mixed up Re the forms and the blood test results expiring 🙄

Thanks @okokok000. I had 3 months in my head but clinic are having me re-do them and it’s under 3 months from the last which is why I’m doubting myself - have left them a message to confirm
