21 day blood results - help!

Day 21 bloods have come back - haven't spoken to the doc yet but logged on the health app and results are up. Dr google isn't too helpful and making me panic about the high prolactin levels!! Also don't think I've ovulated this month?

Progesterone 20.2
LH 4.7
FSH 5.8
TSH 4.26
Prolactin level 892

I'm afraid I don't have the answers, but I just wanted to share my experience: my TSH was a bit higher than yours (around 4.6-4.7) and the fertility consultant I spoke to prescribed thyroxine to bring it to below 2.5 for fertility reasons.
Unfortunately the GP didn't pick that up, but I would ask if I were you.

@confuzzzzzled prolactin should be under 440ish but you can't go on one result - stress can cause it to rise (even rushing to the appointment) so you need to have it repeated. If it remains consistently high it can easily and quickly be brought down with medication.

As PP says TSH should be under 2.5 but the NHS will tell you that under 4 is normal so you might have to push them a bit!

And I don't know anything about the FSH and LH at that point during the cycle.

Progesterone is low but how long are your cycles as day 21 isn't right for everyone.

Who ordered such tests?. Do you have a diagnosis?. I would go all out to get one of these now.

LH and FSH should be measured on day 2, not day 21.

Its usually progesterone solely that is measured on day 21 (which is a standard blood test)

Have your thyroid levels been checked properly with regards to both prolactin and TSH?.

Thanks all! Prolactin level is what's concerned me, but part of me is hoping that that is the issue and it's a quick fix. My periods are horrendous and seem to be getting mysteriously shorter, so that paired with 2 years TTC prompted the doc to order bloods. They initially wanted me to get my general hormone levels checked and for me to come back for day 21 bloods, but my blood test date ended up landing on day 21 so I got them all at once.

Who requested the blood test?.

Are you going to be referred to a gynaecologist?. Has this already happened?. Better to be referred than for a doctor to potentially mess around in ignorance.

Textbook symptoms of endometriosis. I've also been referred through to the hospital for ultrasounds/scans so I'm guessing after getting my blood results and scan results I'll be referred to a gynaecologist?

I have had more internal ultrasounds than I care to mention and NONE of those tests including blood tests ever detected the vast amount of endometriosis that was in my uterine cavity. It was only found during a keyhole surgery type op called a laparoscopy.

You need a referral to a gynae asap; do not let this get further delayed via ultrasound scans and the like.

Below 30 nmol/L progesterone indicates ovulation hasn't taken place within the last 7 days on CD21, however if your cycles are longer or shorter timing can be everything. My cycles are irregular, currently 35ish days and I had low progesterone on CD21 but a repeat on CD28 indicated I had ovulated sometime in the past week.

Also adding to other posters that say that the other hormones (FSH, prolactin in particular) should be tested on CD3, not CD21. CD21 is only useful for looking at ovulation

You should speak to your GP and get repeats set up for CD3, CD21 (and CD28 repeat etc if your cycles are longer than 28 days)

Perfect 28 day cycles!! Which is why I'm pretty bummed at the progesterone levels

It's possible you just had an anovulatory cycle and bad luck, and you could be irregularly ovulating rather than not ovulating at all. I know how awful it must feel but its definitely worth repeating the CD21 test to see for sure.
