Implantation failures !!

Hi Ladies,

Need advice pls.. i have had 2 transfers so far with top grade embryo and both resulted in bfn due to implantation failure.. i have been trying naturally before ivf and have unexplained infertility. Before lockdown, got hysteroscopy done too and all was normal.. i am 34 years with hypothyroid (tsh 3.5) and PCO (not PCOS) and no child.. have been trying for 3 years now and dont know where things are going wrong ! At the moment i am trying naturally till ivf resumes and still getting BFN.. is there anything i can do to improve implantation chances ?? Please advice

I do know that drinking wine significantly reduces the chances of success.
A lot of people try everything diet wise from Brazil nuts, sweet potatoes etc but does it work? Probably not but it may make you feel like you are doing something to help your chances. Good luck

I will admit that i drink wine but occasionally and in a minimal quantity. Also i dont take wine in 2WW - though not sure if it helps by restricting in a small window only.

Hi i have had failed to conceive and had a number of early MC before we went down the IVF route and they have no real reason for it but on my last consultant myself and the consultant came to the conclusion that i probably have lining issues and there for needed to improve implantation. With myself i take a varied amount of things to help naturally and also with FET we did implant and was he strongest i have ever seen a BFP total dye stealer but sadly lost at 7 weeks. I have taken the herbal Brazil nuts, Raspberry leaf tea, L-argnate, Vit D, Vit C,Vit e and low dose aspirin. I also cut out caffine.

I am not sure if that helps but i will try these along with the estrogen to help thicken the lining and promote blood flow x

Without knowing if the embryos were genetically "normal" the top grade bit doesn't mean as much as if an embryo is chromosomally normal - but you won't know unless you do IVF with PGS testing

You could also look at ERA to see if maybe you need another day or so on progesterone to make the best possible uterine environment

TSH should be 2 or under for fertility - I'm surprised your clinic hasn't rectified this.

my TSH was over too and then at 3 and fertility tested further but never did anything about it, just said they would like it lower

Thankyou @MrsLouB for your response. I have been taking most of these meds except aspirin.. how much dose would you suggest and is it to be taken on daily basis or just in 2WW.

How about estrogen dose ? My lining is generally really good (12+) ... thats what i have heard from fertility nurses over last two transfers but somewhere after than things go wrong for me 🙄

@ivfgottostaypositive thanks for your response. I am currently on nhs funded cycle with Nuffield Woking clinic and havent heard about these options from them.. once clinic reopens will surely check with them ..

@CeibaTree Thanks for your response. My consultant did say that they would like it below 2 but also said that it is not must.. what i am not sure is if its their ignorance or it is really not must !! I am with Nuffield Woking and on NHS funded cycle so chances are that i am ignored 😐

Is your clinic strict about having TSH below 2 ? I tried proactively to work with my GP to increase dose to reduce TSH but my T4 goes out of range then.. hate thyroid !

Hi @duckhouse my consultant just said one a day if the lose dose aspirin so below 100 usually has the heart sign on the pack and i took from day 1 of period but he said can do that or from day 7 also the selenium supplement is good cant remeber if that is the one in brazil nuts or pineapple but can all help with implantation xx something helped on the last fet but failed most probably due to embryo we just dont know. But if your lining is good your wont need the estrogen are you on progesterone supplement x

Hi @duckhouse I'm not with a clinic but the endocrinologist who I was seeing at one point said that under 2 was an absolute must for optimal fertility. Are you on thyroxine now?

Ah sorry just saw you are! Hmm that's a tricky one. I'm not sure what T4 levels should be and how important they are for fertility as opposed to TSH. I have Hashimoto's and I find that cutting out gluten and sugar really helps if my TSH starts creeping up.

@CeibaTree yes I am on Levothyroxine 50mg daily for more than an year now and my TSH has stabilised. I am thinking to increase my dose to 75 and see if TSH comes down further. How much is your dose ? My GP is treating me on hit n trial and is not an expert... neither she cares to transfer me to endo

@MrsLouB no i am not on progesterone supplement at the moment trying naturally. When i was being treated for Ivf then clinic had put me on progesterone pessaries which is a standard thing they do for all. I have heard about baby aspirin and might try it too this time

@duckhouse I'm on 100 during the week and 125 at the weekends. I think I was just lucky as when I got diagnosed I had a GP who was very clued up. If yours is happy to tweak the dose, and you are having regular blood tests, I'd ask to go straight to 100 and see if that gets your TSH down. The endocrinologist I saw when I was pregnant with my son really did stress if I was planning to have another child it was important to have my TSH at 2 or under for optimal fertility. Maybe he was just over cautious though, who knows!

@CeibaTree @duckhouse my IVF consultant said the same - TSH should be under 2. Mine was 2.8 so they put me on levothyroxine.

As other posters have suggested you could look into ERA. Also my new clinic were going to give me prednisolone at the point of embryo transfer. I also had mini thrombophilia screen (which is a couple of blood tests) which looks for blood clotting issues

Thanks @CeibaTree for sharing details on the dose. May i ask how soon blood test should be done to see the impact of the medicine ? My GP says test within 6 weeks should show difference.: do you agree based on your experience with Endo ?

Thanks @Wesstywoo .. i will check with my consultant once IVF resumes if she would consider these tests for me.. and will use the gap time to get my thyroid in place.. how much is your dose ? And how long did it take for you to get below 2 ?

@duckhouse I was on 50 and it came down pretty quick - around 3 weeks, however be sure to have regular blood tests (every 6 weeks or so). I swapped over clinics so ended up missing a couple of tests and by then it had come down way too low so it is important to keep a check on it!

@duckhouse From what I can remember when my GP was working out my dose (back in 2012) I had blood tests every 3 weeks, but during my pregnancies I had thyroid blood tests every 6 weeks. My dose actually didn't change at all during pregnancy, so I think once you are on the perfect dose for you, you are on the right dose in general if that makes sense :)

I did an ERA test after 3 failed transfers and my 4th was a success. Not sure if it helped but might be useful to look into this. It's an endometrium biopsy to see if it's receptive. The test will give you a personalized transfer window.

@duckhouse I have had three unsuccessful rounds of IVF, and waiting to do my fourth. I'm with Create as I have a low AMH, and after my third was unsuccessful they suggested a new range of blood tests. The results showed that I have a genetic mutation affecting one copy of a double gene that regulates blood clotting. That suggests that there is a minimal chance that my blood is clotting hence the implantation failure. Both my mum and my maternal grandmother had multiple miscarriages, and I've never had a pregnancy. Could you investigate these blood tests? Create call it the recurrent miscarriage test.
