PCOS? Blood test confirm no ovulation, other symptoms include..

Day 3 blood test and 21 day blood test confirm I didn't ovulate last cycle.

My cycles average 42-47 days and my periods are really heavy. I have low level twinging in my pelvic area all the time but tends to be worse latter half of the cycle. Beard, moustache, hairy generally. Slightly overweight.

NHS gynae appointment is a 4-6 month wait. I'm tempted to go for a private ultrasound to see if they can see anything there.

I'm still feeling a bit gut punched and confused about it all.

A day 3 test is irrelevant for ovulatation...

What were you fsh and lh taken on day 3?

No idea to be honest, my GP didn't say (I don't know what fsh is, off to Google) just that my tests were "negative" and "no ovulation on this cycle" and to just wait for the gynae appt.

Lh is all over the place from OPKs, several "peaks" throughout the cycle but no positive ever. Same the cycle before.

I was a bit flustered at the time, GP was running late and wanted to move onto the next patient. I should have questioned and been prepared but I wasn't, kicking myself now.

What cycle day did you do the cd21 test? It should be 7 days before af arrived, so if you had a 47 day cycle, it should have been done around cd39/40.

I'd call and ask for the numbers for both blood tests.

My GP sent me for a pelvic scan as part of my investigations, so might be worth asking. I know each area is different.

I have PCOS and started taking Myo Inositol in December. It brought on AF 2 weeks after starting and next cycle I got my first ever positive OPK (temp confirmed also).

If the wait is 4-6 months I'd try it and by the time your appointment comes round you may be further forward than you are now.

GP told me to go on CD25 (or 7 days prior) and I ended up going on CD21 Venus I'm going to ring shortly and see if I can get the numbers.

That's definitely worth a go apple I'll start researching that now.

Do not use OPKs if PCOS is present as these can give false positive readings. These kits read LH (luteinising hormone) and many PCOSers have an excess of this hormone therefore the kit reads that.

LH and FSH should have been tested and compared against each other re the day 3 test. Re the day 21 test this measures progesterone and a number of 30 or over would indicate ovulation.
Its a standard test and I would think having it done later would have made no real difference to the overall result.

I've not even got a diagnosis, nowhere near to be honest, the GP wasn't particularly helpful (or perhaps not knowledgeable) and that threw me off. I'm just floundering and clutching at straws a bit.

Ok so, Day 3 tests FSH 5ui, LH 3.5ui, prolactin 137mui. Day 21 progesterone: 1 nmol. So I know that my progesterone level is basically dead in the water there. And fsh and Lh are also low?

Glad I got the numbers, thank you for encouraging me to do that.

I actually couldn’t get a gynae appointment because at the time I wasn’t trying for a baby so didn’t fit the criteria.

But I experience a lot of the same symptoms OP

I think your fsh and lh levels are within the normals ranges. I'm not sure about prolactin - that seems high? @PennyArrowBar

With regards to progesterone, if you had it done that early in your cycle, that would explain why it's low. You could ask to have the cd21 test redone and go when you think you're 7 days before your period. It might still show no ovulation (if you're not ovulating) but might not.

Have you ever tried temping?

How long was that specific cycle you did the ovulation test on?
Tbh with varying cycles its very hard to get the right days.
And i then thought i had a short luteal phase anyway making it harder.

I think i would have a scan done. They can see if you are producing loads of eggs. And although that isnt probably conclusive add that to your long and variable cycles.
Maybe look at weight loss if necessary.

And have you had thyroid levels checked as that should be a tsh of around 1 for fertility.

Good point about the thyroid @Awkward1 mine fluctuates so I've been prescribed levothyroxine now.

I also used Myo Instanol (Amazon!) but I didn't find it had much effect. I then was placed on Metformin (500mg 3X day) it can make you feel sick but I had a period the following month and after 3 cycles we got a BFP and to top it off most of my cysts have gone - I only have one left on my right ovary (as of my 12 week scan)
