ivf2-did your existing clinic change anything?

I’ve signed up for multi cycle with clinic to which first ivf fetsx2 ended up being chemical. I’ve learnt today clinic are doing exact same thing, Now I’m not sure if it’ll work and waste of time and money.

Anyone had exact same thing with same clinic to have success?

@Kitkat369 it's so hard to know what to do for the best - my clinic added DHEA for cycle 2 (unsuccessful) and have gone elsewhere for a second opinion ahead of cycle 3 and they said actually that's detrimental and possibly causing enlarged antral follicles - so who is right?! What was your protocol the first time and how did you respond? What were you on in terms of luteal support?

My fet1 I was on lubion, progesterone and clexane. Second fet the same plus prednisone. I’d hoped from my wtf appt the consultant would change it up and she mentioned heparin but seems like it’s exact same thing as clexane so now I feel dubious it’ll work.

I don't think clinics tend to change anything for transfers until after the 3rd one and then that tends to be when you don't start out on all the drugs eg clexane or prednisolone

As you've had 2 chemicals they will argue that since you did get implantation that it's unlikely to be anything medication related and will be down to embryo issues

I think another thing frustrating me is during the wtf appointment Consultant mentioned heparin for next transfer, I was on clexane and so thought great a change but seems they are same thing and yet she failed to tell me that. My confidence in them is dropping. I’m wondering if I should just cut my losses and go elsewhere though I paid for multicycle

My clinic has changed from short protocol to flare protocol and my drugs have been changed from Menopur and Gonal-F to Fostimon and Meriofort. We had egg quality issues on our first round.

Part of the reason I'm changing clinics is because they didn't throw all they could at my previous cycles and I'm sure cycle 1 failed because of it - have decided to try natural modified ivf this time as I think I need a completely different approach

@itwasalovelydreamwhileitlasted How are you getting on with natural modified at Create? We have no more embryos left from our three cycle mild package, and they've mentioned natural modified as an option. The Dr mentioned that we would probably only get one egg from natural modified, but I'm sure on a different thread you got two from a cycle?

I'm finding it much better - so out of the 3 cycle package I've done 2 so far with the final one next month and will transfer with that one

Cycle 1 - pure natural modified - tamoxifen tablets daily from cycle day 3 and 150ml Bemfola every other day. We got 2 eggs - BOTH fertilised (which is great for us had less than 40% fertilise on normal IVF). On day 3 both were Grade 1 (also never had that before!) - they froze one on day 3 and then I wanted to get at least one blastocyst banked - the other one was frozen as a day 5 3BB

Cycle 2 - slight tweak to see if could get the eggs which didn't grow cycle 1 to grow. So still tamoxifen but Bemfola was changed to daily. From cycle day 5 though it was reduced to 75ml daily with 75ml luveris
We got 7 eggs - 6 fertilised normally. Day 3 all around Grade 2. Day 5 which is today they've frozen a 4aa (which is the best we've ever had!) and there is another which is a bit slow which they are going to check tomorrow.

Cycle 3 - I'm guessing will stay the same as cycle 2

After so many egg collections I truly believe that maybe my body isn't designed to make more than 2 blastocysts per cycle. Doesn't matter if they collect 14 eggs or 2. I always end up with 2!

The quality of eggs (almost 100% fertilisation rate) and blastocysts though doing natural modified so far is much better

Wow that's amazing - you don't hear of people doing natural modified so much on these boards. I have very low AMH so I'm hoping for 1 egg per cycle and any more than that is a bonus! Good luck for cycle 3 xx

Yeah I only got 1 last cycle when I blasted my ovaries with max dose stims + DHEA so if I'm only ever going to get 1 I need to focus on quality.

I was thinking of cycle 2 doesn’t work maybe using create fir milder ivf as my amh is about 6. Is that similar to natural modified?

Mild ivf is just another name for short protocol - so no down reg - you tend to get fewer eggs (although I did 2 cycles and got the same number of eggs as friends on full ivf)

The only real difference with natural modified is you take tamoxifen tablets which has a similar effect to clomid and then only do stims every OTHER day (you also start a day later in your cycle - day 3 not 2) and also likely to get fewer eggs again - they are aiming to get between 1 and 7 eggs depending on amh/Antral follicle count

Natural modified is really for women who don't necessarily ovulate every month but Create were happy to put me on it to focus on collecting fewer eggs to see if it would improve quality

Ahh ok, so with my endo not sure if would be suitable as they need my system to quieten down before stims

@itwasalovelydreamwhileitlasted Wow it sounds like this has been a much better fit for you. And great news on the quality. That's my issue for sure, so I'm hoping I'll get better results next cycle. I'm waiting for Create to get back to me to let me know what they recommend for the next cycle. I'll be starting at the beginning of March, so not long after your last cycle.

I had short protocol twice then mild IVF and they definitely aren’t synonymous. Mild IVF was a clomid equivalent and low dose stims every other day.
